摘要:
目的：比较保乳术与改良根治术两种术式治疗乳腺癌的近期疗效及术后美观度。方法：对2014年10月-2017年10月笔者医院收治的95例乳腺癌患者的临床资料进行整理，根据手术方法不同分为对照组（49例，改良根治术）与观察组（46例，保乳术）。对比两组手术指标、近期疗效、术后美观度、手术前后生活质量及术后满意度情况，评估两种手术方法的安全性，比较术后3年生存率及预后效果。结果：观察组手术时间、住院时间均短于对照组，术中出血量少于对照组，差异具有统计学意义（P＜0.05）。两组患者术后1个月总有效率、术后3年生存率、局部复发率及远处转移率相近，组间比较差异无统计学意义（P＞0.05）。观察组术后乳房美学效果优良率为89.13%显著高于对照组的18.37%，差异有统计学意义（P＜0.05）。术后6个月，两组社会功能、物质生活、躯体功能、心理功能评分均明显升高，且观察组显著高于对照组，差异有统计学意义（P＜0.05）。观察组术后并发症总发生率10.87%显著低于对照组的55.10%，差异有统计学意义（P＜0.05）。观察组患者满意度问卷评分为（88.65±6.58）分明显高于对照组（70.65±3.21）分，差异具有统计学意义（t=17.107，P＜0.001）。结论：保乳术与改良根治术疗效相当，均可提高患者生存率，但保乳术术后美观度及安全性更佳，且患者满意度高，临床应用价值高。
Abstract:
Objective To compare the short-term efficacy and postoperative aesthetics of breast-conserving surgery and modified radical mastectomy in the treatment of breast cancer. Methods The clinical data of 95 patients with breast cancer admitted to this hospital from October 2014 to October 2017 were sorted out and they were divided into the control group (49 cases, modified radical mastectomy) and the observation group (46 cases, breast-conserving surgery) according to different surgical methods. The surgical indexes, short-term curative effect, postoperative aesthetics, quality of life before and after operation and postoperative satisfaction of the two groups were compared, the safety of the two surgical methods was evaluated, and the 3-year survival rate and prognosis were compared. Results The operation time and hospital stay of the observation group were shorter than those of the control group, and the amount of intraoperative bleeding was less than that of the control group, the differences were statistically significant (P ＜0.05). The total effective rate at 1 month, 3-year survival rate, local recurrence rate and distant metastasis rate of the two groups were similar, and there was no significant difference between the two groups (P＞0.05). The excellent and good rate of postoperative breast beauty in the observation group was 89.13%, significantly higher than that in the control group (18.37%, P＜0.05). Six months after operation, the scores of social function, material life, physical function and psychological function in the two groups were significantly higher, and the scores in the observation group were significantly higher than those in the control group (P＜0.05). The total incidence of postoperative complications in the observation group was 10.87%, significantly lower than that in the control group (55.10%, P＜0.05). The score of satisfaction questionnaire in the observation group was (88.65±6.58), which was significantly higher than that in the control group (70.65±3.21, P＜0.05). Conclusion Breast conserving surgery has the same curative effect as modified radical mastectomy, and can improve the survival rate of patients. However, breast conserving surgery has better aesthetics and safety, high patient satisfaction and high clinical application value.
