摘要:

目的：对比上颌中切牙区即刻与延期种植的唇侧骨量情况及美学效果。方法：选择2016年10月-2020年1月在南京医 科大学附属淮安第一医院行上颌中切牙区单颗牙种植的86例患者，依据治疗方式的不同分为即刻组（43例）和延期组（43 例）。对比两组患者种植体成功率、唇侧骨量、美学效果及满意度。结果：即刻组和延期组种植成功率分别为97.67%、 95.35%，两组种植体成功率比较，差异无统计学意义（P=0.557）。两组患者种植即刻1 mm位点、10 mm位点唇-腭侧牙槽嵴厚 度、根方及冠方骨板厚度对比，差异均无统计学意义（P＞0.05）；12个月后，两组患者即刻1 mm位点、10 mm位点唇-腭侧牙 槽嵴厚度均降低，但即刻组1 mm位点、10 mm位点唇-腭侧牙槽嵴厚度高于延期组，差异有统计学意义（P＜0.05）；两组患者 12个月后根方、冠方唇侧骨量对比，差异无统计学意义（P＞0.05）。即刻组种植修复即刻、12个月后红色美学指数（Pink esthetic score,PES）评分、白色美学指数（White esthetic score,WES）评分均显著高于延期组，且满意度均显著高于延 期组，差异均有统计学意义（P＜0.05）。结论：两种种植技术种植成功率相近，与延期种植相比，即刻种植更有利于缓解 牙槽骨吸收，提高美学效果及患者满意度。

Abstract:

Objective To compare the clinical effects of immediate and delayed implants on the labial bone volume and aesthetics in the maxillary central incisor area. Methods From October 2016 to January 2020, 86 patients who underwent single dental implantation in the maxillary central incisor area at the Huai’an First Hospital affiliated to Nanjing Medical University were selected as the research objects. According to the diff erent treatment methods, they were divided into immediate groups (43 cases) ), deferred group (43 cases). The implant success rate, labial bone mass, aesthetic effect and satisfaction degree were compared between the two groups. Results The implantation success rates of the immediate group and the delayedgroup were 97.67% and 95.35%, respectively. The implant success rates of the two groups were compared, and the diff erence was not statistically signifi cant (P=0.557). There was no signifi cant diff erence in the thickness of the lip-palatal alveolar ridge, the thickness of the root and coronal bone plates between the two groups of patients at 1 mm site and 10 mm site immediately after implantation (P＞0.05). After 12 months, the thickness of the lip-palatine alveolar ridge at the 1 mm site and 10 mm site of the two groups of patients decreased (P＜0.05), but the 1 mm site and the 10 mm site lip-palatine teeth in the immediate group The thickness of the groove ridge was higher than that of the delayed group (P＜0.05). There was no statistically signifi cant diff erence between the two groups of patients in the root and crown labial bone mass after 12 months (P＞0.05). The scores of Pink Aesthetic Index (PES) and White Aesthetic Index (WES) of the immediate group immediately after restoration and 12 months later were signifi cantly higher than those of the postponement group (P＜0.05). The satisfaction of patients in the immediate group immediately after repair and 12 months later was signifi cantly higher than that in the delayed group (P＜0.05). Conclusion The implantation success rates of the two implant techniques are similar. Compared with delayed implantation, immediate implantation is more conducive to alleviating alveolar bone resorption, improving the aesthetic eff ect and patient satisfaction.
