摘要:
目的:探讨活性因子敷料结合负压封闭引流(Vacuum sealing drainage,VSD)对深Ⅱ度烧伤创面愈合后瘢痕挛缩 畸形的影响。方法:选取2018年6月-2021年3月收治的120例深Ⅱ度烧伤创面愈合后瘢痕挛缩畸形患者为研究对象,采用 随机数字表法分为观察组60例和对照组60例。对照组采用常规方法并VSD治疗,观察组在对照组治疗基础上联合外用活性 因子敷料,两组均持续治疗14 d。治疗结束后,评估两组治疗效果,采用酶联免疫法检测血清白介素-10(Interleukin6,IL-10)和细胞间黏附分子(Intercellular adhesion molecule 1,ICAM-1)水平,采用放射免疫法检测血清表皮生长 因子(Epidermalgrowth factor,EGF)水平;比较两组治疗后1、3、6个月的瘢痕厚度。采用温哥华瘢痕量表(Vancouver scar scale,VSS)评估两组治疗前及治疗1、3、6个月烧伤部位瘢痕修复情况。结果:观察组治疗有效率为91.67%,高于对照 组的78.33%(P<0.05);观察组患者治疗后1、3、6个月的瘢痕厚度均低于对照组(P<0.05);治疗后,观察组IL-10和EGF水平高 于对照组,ICAM-1低于对照组,差异均有统计学意义(P<0.05);治疗后1、3、6个月,观察组VSS评分均低于对照组(P<0.05)。 结论:活性因子敷料结合VSD可提高深Ⅱ度烧伤创面愈合后瘢痕挛缩畸形疗效和瘢痕瘢痕挛缩畸形改善效果,安全性高。
Abstract:
Objective To explore the effects of active factor dressing combined with wound vacuum sealing drainage (VSD) on scar contracture deformity after wound healing in deep degree-II burn. Methods A total of 120 patients with scar contracture deformity after wound healing and deep degree-Ⅱ burn admitted to the hospital were enrolled as the research objects between June 2018 and March 2021.According to random number table method,they were divided into observation group (60 cases) and control group (60 cases).The control group was given routine treatment and VSD,while observation group was given active factor dressings on basis of control group.All were continuously treated for 14 d.After treatment,curative effect in both groups was evaluated.The levels of serum interleukin-10 (IL-10) and intercellular adhesion molecule 1 (ICAM-1) were detected by enzyme-linked immunosorbent assay.The level of serum epidermal growth factor (EGF) was detected by radioimmunoassay. At1,3 and 6 months after treatment,scar thickness was compared between the two groups. All were followed up for 6 months after treatment. The scar repair at burn sites in both groups was evaluated by Vancouver Scar Scale (VSS) before treatment,at 1,3 and 6 months after treatment. Results The response rate of treatment in observation group was higher than that in control group (91.67% vs 78.33%) (P <0.05).At 1 month,3 months and 6 months after treatment,scar thickness in observation group was thinner than that in control group (P <0.05). After treatment,levels of IL-10 and EGF in observation group were higher than those in control group,while ICAM-1 was lower than that in control group (P <0.05).At 1 month,3 months and 6 months after treatment, VSS score in observation group was lower than that in control group (P<0.05). Conclusion Active factor dressing combined with VSD can improve curative eff ect and improvement effect of scar contracture deformity after wound healing in deep degree- Ⅱ burn,with high safety.
