摘要:
目的:探讨中药浸泡联合5-氨基酮戊酸光动力疗法(5-aminolevulinic acid photodynamic therapy,ALA-PDT)治疗 跖疣的疗效及其对调节性T细胞、转化生长因子-β(Transforming growth factor-β,TGF-β)、γ-干扰素(Interferonγ,IFN-γ)的影响。方法:按照随机数字表法将2019年1月-2020年12月笔者医院收治的160例跖疣患者分为对照组和观 察组,每组80例。对照组使用ALA-PDT(每隔20天1次,共3次),观察组使用中药浸泡(每天1次)联合ALA-PDT。比较两组的 临床疗效、疣体评分、外周血CD4+ CD25+ CD127dim/-Treg水平、血清TGF-β、IFN-γ水平,不良反应及复发率。结果:观察 组总有效率92.50%,明显高于对照组80.00%,差异有统计学意义(P<0.05)。治疗后,观察组的疣体评分[(0.39±0.0.4) 分 vs (1.05±0.16) 分]、外周血CD4+ CD25+ CD127dim/-Treg水平[(6.90±1.04)×105 个 vs (8.72±2.58)×105 个]及血清 TGF-β[(0.75±0.16)ng/ml vs (1.02±0.21)ng/ml] 明显低于对照组,血清IFN-γ水平[(5.63±1.14)ng/L vs (4.09±1.02)ng/L]明 显高于对照组,差异均有统计学意义(P<0.05)。观察组的复发率(5.00%)明显低于对照组(18.75%),差异有统计学意义 (P <0.05)。两组不良反应发生情况比较,差异无统计学意义(P>0.05)。结论:中药浸泡联合ALA-PDT治疗跖疣疗效显著, 安全性好,能有效调节免疫-炎症反应,降低复发率。
Abstract:
Objective To investigate the eff ects of Traditional Chinese Medicine(TCM) soaking combined with 5-aminolevulinic acid photodynamic therapy(ALA-PDT) on plantar warts and its effects on regulatory T cells, transforming growth factorβ(TGF-β) and interferon-γ(IFN-γ). Methods According to random number table method, a total of 160 patients with plantar warts who were divided into control group and observation group in the author’s hospital from January 2019 to December 2020, with 80 patients in each group. The control group was given ALA-PDT(once every 20 days, three times in total), and the observation group was given TCM soaking(once a day) combined with ALA-PDT. The clinical effi cacy, warts score, peripheral blood levels of CD4+ CD25+ CD127dim/-Treg , serum levels of TGF-βand IFN-γ, adverse reactions and recurrence rates were compared between two groups. Results The total eff ective rates(92.50% vs 80.00%) in the observation group was signifi cantly higher than that in the control group(P <0.05). The warts score[(0.39±0.0.4)points vs (1.05±0.16)points], peripheral blood levels of CD4+ CD25+ CD127dim/-Treg [(6.90±1.04)×105 vs (8.72±2.58)×105 ],the serum TGF-β [(0.75±0.16)ng/ml vs (1.02±0.21) ng/ml] was signifi cantly lower than that of the control group.The serum levels of IFN-γ[(5.63±1.14)ng/L vs (4.09±1.02) ng/L] in the observation group was significantly higher than that in the control group(P <0.05). The recurrence rates (5.00% vs 18.75%)in the observation group was signifi cantly lower than that in the control group(P <0.05). There was no signifi cant diff erence in adverse reactions between the two groups(P >0.05). Conclusion TCM soaking combined with ALAPDT had signifi cant eff ect and good safety on plantar warts. It could eff ectively regulate the immune- infl ammatory response and reduce the recurrence rates.
