摘要:
目的:观察超脉冲点阵CO2激光联合皮下分离术治疗萎缩性痤疮瘢痕的疗效及不良反应。方法:将64例萎缩性痤疮瘢 痕患者随机分为试验组(32例)和对照组(32例)。试验组首先对萎缩性痤疮瘢痕皮损行皮下分离术,随后进行超脉冲点阵 CO2 激光治疗,每2个月治疗一次,共治疗3次。对照组仅予超脉冲点阵CO2激光治疗。结果:治疗前两组患者ECCA评分无统计 学意义,治疗后试验组ECCA评分显著低于对照组,下降指数高于对照组。V形、U形、W形瘢痕治疗前后ECCA评分比较差异均 有统计学意义;治疗后试验组三种类型痤疮瘢痕ECCA评分均低于对照组,下降指数优于对照组(P<0.05)。试验组治疗有 效率为93.75%,明显高于对照组(75.00%)。三种类型痤疮瘢痕治疗有效率比较,试验组V形痤疮瘢痕有效率高于对照组, 差异具有统计意义(P<0.05);两组U形及W形痤疮瘢痕治疗有效率差异无统计学意义(P>0.05)。试验组患者疼痛指数平 均值为5.28±0.96,高于对照组的4.75±1.34(P<0.05)。试验组不良反应发生率(25.00%)和对照组(21.88%)比较差 异无统计学意义(P>0.05)。结论:超脉冲点阵CO2激光联合皮下分离术具有良好的临床疗效,可有效改善萎缩性痤疮瘢痕 的症状,不良反应轻微,是治疗萎缩性痤疮瘢痕有效治疗手段。
Abstract:
Objective To observe the effi cacy and adverse reactions of ultrapulse fractional CO2 laser combined with subcision in the treatment of atrophic acne scars. Methods Sixty-four patients with atrophic acne scars were randomly divided into an experimental group (32 cases) and a control group (32 cases). The experimental group underwent subcision of the atrophic acne scars followed by ultrapulse fractional CO2 laser treatment every 2 months for a total of 3 sessions. The control group received only ultrapulse fractional CO2 laser treatment. Results There was no signifi cant diff erence in the ECCA scores between the two groups before treatment. After treatment, the ECCA score in the experimental group was signifi cantly lower than that in the control group, and the reduction index was higher than that in the control group. The diff erences in ECCA scores before and after treatment were statistically signifi cant for V-shaped, U-shaped, and W-shaped scars. After treatment, the ECCA scores for all three types of acne scars in the experimental group were lower than those in the control group, and the reduction index was better than that in the control group (P<0.05). The effective rate of treatment in the experimental group was 93.75%, signifi cantly higher than that in the control group (75%). When comparing the eff ective rates of treatment for the three types of acne scars, the eff ective rate for V-shaped scars in the experimental group was higher than that in the control group, with a statistically signifi cant diff erence (P <0.05). There was no signifi cant diff erence in the eff ective rates of treatment for U-shaped and W-shaped scars between the two groups (P >0.05). The average pain index in the experimental group was 5.28±0.96, which was higher than the control group 4.75±1.34 (P<0.05). There was no signifi cant diff erence in the incidence of adverse reactions between the experimental group (25.00%) and the control group (21.88%) (P >0.05). Conclusion Ultrapulse fractional CO2 laser combined with subcision has good clinical effi cacy and can eff ectively improve the symptoms of atrophic acne scars. The adverse reactions are mild. It is an eff ective treatment method for atrophic acne scars.
