目的:分析超脉冲点阵CO2激光联合窄谱强脉冲光(Dye pulse light,DPL)治疗创伤后早期增生性瘢痕的效果。方法:选择2018年12月-2020年9月于笔者医院就诊的86例创伤后增生性瘢痕患者为研究对象,根据治疗方法不同将患者分为观察组45例和对照组41例。对照组采用超脉冲点阵CO2激光治疗,观察组在对照组基础上联合DPL治疗。比较两组患者治疗前和治疗后3个月视觉模拟评分法(visual analogue scale,VAS)和温哥华瘢痕量表(Vancouver scar scale,VSS)的评分,比较两组患者的临床疗效;随访至治疗后1年,比较两组患者的瘢痕美容评估评级量表(Scar cosmesis assessment and rating,SCAR)评分及不良反应发生情况。结果:两组患者一般临床资料、治疗前VAS、VSS、SCAR评分及治疗后不良反应发生率比较差异均无统计学意义(P>0.05),治疗后3个月,两组患者VAS和VSS评分均明显下降,其中观察组均低于对照组,观察组患者临床疗效优于对照组(95.56% VS. 80.49%),治疗后1年观察组SCAR评分显著低于对照组,以上差异均有统计学意义(P<0.05)。结论:超脉冲点阵CO2激光联合DPL治疗早期创伤后增生性瘢痕患者疗效确切,患者满意度较高,值得临床推广应用。
Abstract:
Objective To analyze the effect of ultra-pulsed fractional CO2 laser combined with dye pulse light (DPL) in the early treatment of post-traumatic hypertrophic scar. Methods 86 patients were prospectively enrolled and divided into observation group (n=45) and control group (n=41) according to different treatment methods. The control group was treated with ultra-pulsed fractional CO 2 laser, and the observation group was treated with DPL on the basis of control group. The scores of visual analogue scale (VAS) and Vancouver scar scale (VSS) before treatment and 3 months after treatment were compared between the two groups, the clinical efficacy was compared between the two groups.The patients were followed up until 1 year after treatment, and the scar cosmetic assessment rating scale (SCAR) score and the occurrence of adverse reactions were compared between the two groups. Results There was no significant difference in the general clinical data, VAS, VSS, and SCAR scores before treatment and the incidence rate of adverse reactions after treatment between the two groups (P >0.05). Three months after treatment, the VAS and VSS scores of the two groups were significantly decreased, of which the observation group was lower than the control group, the clinical efficacy of the observation group was better than that of the control group (95.56% vs. 80.49%), one year after treatment, the SCAR score in the observation group was significantly lower than that in the control group, and the above differences had statistical significance (P<0.05). Conclusion Ultra -pulsed fractional CO2 laser combined with DPL is effective in the treatment of patients with early post-traumatic hypertrophic scar, with high patient satisfaction, and it is worthy of clinical application.
