摘要:
目的：研究痤疮患者血清中辅助性T细胞1（T helper cell 1,Th1）和辅助性T细胞17（T helper cell 17,Th17）活 化与病情严重程度的关系。方法：选取2019年6月-2021年6月笔者医院就诊的143例痤疮患者，根据Pillsbury法将其分为轻 度48例、中度55例及重度40例。比较不同患者病情严重程度、一般资料、Th1细胞和Th17细胞水平。采用Pearson相关系数分 析Th1细胞和Th17细胞水平与病情严重程度的关联性，采用Logistic回归分析重度痤疮的影响因素。结果：不同病情严重程 度痤疮患者年龄、家族史、治疗情况比较，差异无统计学意义（P＞0.05）；重度痤疮患者男性、饮食喜好油腻或辛辣、吸 烟或喝酒、每天使用手机或电脑时间、皮肤类型为油性占比大于轻中度患者，痤疮初发年龄、每日睡眠时间小于轻中度患者 （P＜0.05）；随着痤疮病情程度的加重，Th1细胞和Th17细胞水平呈逐渐升高趋势（P＜0.05）；经Pearson相关性分析，痤 疮患者Th1细胞和Th17细胞水平与病情严重程度呈正相关（P＜0.05）；经Logistic回归分析，性别、痤疮初发年龄、饮食喜 好油腻或辛辣、吸烟或喝酒、每日睡眠时间、每天使用手机或电脑时间、皮肤类型油性、Th1细胞及Th17细胞均为重度痤疮 的影响因素（P＜0.05）。结论：痤疮患者病情严重程度与Th1细胞和Th17细胞水平呈现密切相关性，且为重度痤疮发生的影 响因素。研究Th1细胞和Th17细胞可为痤疮的病情进展机制研究及临床治疗提供新思路。
Abstract:
Objective To study the relationship between the activation of T helper cell 1 (Th1) and T helper cell 17 (Th17) in patients with acne,and analyze their correlation with the severity of illness. Methods One hundred and forty-three patients with acne in our hospital from June 2019 to June 2021 were selected and divided into 48 cases of mild, 55 cases of moderate and 40 cases of severe according to Pillsbury method. The general data of diff erent disease severity and Th1 cells and Th17 cells were compared. Pearson correlation analysis was used to analyse the association between Th1 and Th17 cell levels and severity of the condition, and logistic regression was used to analyze the infl uencing factors of severe acne. Results There was no signifi cant diff erence in age, family history and treatment among acne patients with diff erent severity (P ＞0.05). The patients with severe acne, male, the preference for greasy or spicy diet, smoking or drinking, the time of using mobile phone or computer every day, and the skin type of oily accounted for more than those with mild and moderate acne, and the initial age of acne and daily sleep time were less than those with mild and moderate acne (P＜0.05). With the aggravation of acne, Th1 cells and Th17 cells increased gradually (P＜0.05). Pearson correlation analysis showed that Th1 cells and Th17 cells were positively correlated with the severity of acne (P＜0.05). By logistic regression analysis, gender, initial age of acne, dietary preference, greasy or spicy, smoking or drinking, daily sleep time, daily use of mobile phone or computer, skin type, oily, Th1 cells and Th17 cells were the infl uencing factors of severe acne (P ＜0.05). Conclusion The severity of acne patients is closely related to Th1 cells and Th17 cells, and they are the infl uencing factors of severe acne, which can provide new ideas for the study of the mechanism of acne progression and clinical treatment.
