[摘要]目的:探讨无托槽隐形矫治器与直丝弓矫治器对正畸患者牙周指数、变异链球菌和牙龈卟啉单胞菌的影响。方法:
选取2017年3月-2019年3月在笔者医院口腔科接受正畸治疗的108例患者为研究对象。根据随机数字法将患者分为对照组和
观察组,每组54例。对照组采用直丝弓矫治器进行治疗,观察组采用无托槽隐形矫治器进行治疗。治疗前及治疗第1、3个
月时检测牙周指标,并分析变异链球菌和牙龈卟啉单胞菌的构成比。 结果:治疗第1个月,两组患者的各项牙周指标与治
疗前比较,差异均无统计学意义(P >0.05);治疗第3个月,两组患者的各项牙周指标均高于治疗前,且对照组菌斑指数
(PLI)、龈沟出血指数(SBI)均高于治疗第1个月,观察组PLI高于治疗第1个月(P <0.05)。治疗第1、3个月,观察组各
项牙周指标均低于同期对照组(P <0.05);治疗第1、3个月,两组变异链球菌和牙龈卟啉单胞菌构成比均高于治疗前,治
疗第3个月,两组变异链球菌和牙龈卟啉单胞菌构成比均高于治疗第1个月(P <0.05)。治疗第1、3个月,观察组的变异链
球菌和牙龈卟啉单胞菌构成比均低于同期对照组(P <0.05)。结论:相较于直丝弓矫治器,无托槽隐形矫治器对正畸患者
具有更佳的牙齿保洁作用,更利于维护牙周健康,减少细菌感染。

Abstract:
Abstract: Objective To investigate the effects of bracketless invisible appliance and straight wire appliance on periodontal
index, streptococcus mutans and porphyromonas gingivalis of orthodontic patients. Methods Total of 108 patients who received
orthodontic treatment in our hospital from March 2017 to March 2019 were selected. According to the random number method,
the patients were divided into the control group (n =54) and the observation group (n =54). The control group was treated with
straight wire appliance, and the observation group was treated with bracketless invisible appliance. Before treatment and at
the 1 and 3 months of treatment, the periodontal indexes of the two groups of patients were detected, and the composition
ratio of streptococcus mutans and porphyromonas gingivalis was analyzed. Results At the 1 month of treatment, there was
no signifi cant difference in periodontal index between the two groups (P >0.05). At the 3 month of treatment, the periodontal
indexes of the two groups were all higher than those before treatment, and the plaque index (PLI) and sulcus bleeding index
(SBI) in the control group were all higher than those in the 1 month of treatment, and the PLI in the observation group were all
higher than those in the 1 month of treatment, and the differences were all statistically signifi cant (P <0.05). At the 1 month of
treatment and the 3 month of treatment, the periodontal indexes in the observation group were all lower than those in the control
group at the same period, and the differences were all statistically signifi cant (P <0.05). At the 1 month of treatment and the 3
month of treatment, the composition ratio of streptococcus mutans and porphyromonas gingivalis in the two groups was higher
than that before treatment. At the 3 month of treatment, the composition ratio of streptococcus mutans and porphyromonas
gingivalis in the two groups was higher than that at the 1 month of treatment, and the differences were all statistically
signifi cant (P <0.05).At the 1 month of treatment and the 3 month of treatment, the composition ratio of streptococcus mutans
and porphyromonas gingivalis in the observation group was lower than that in the control group at the same period, and the
differences were all statistically signifi cant (P <0.05). Conclusion Compared with the straight wire appliance, the bracketless
invisible appliance has a better cleaning effect on the teeth of patients receiving orthodontic treatment, which is more conducive
to maintaining periodontal health and reducing bacterial infection.
