摘要:
目的:探究Meek植皮术联合碳纤维敷料修复大面积烧伤创面的效果。方法:将2018年1月-2020年1月笔者医院就诊的 109 例大面积烧伤患者随机分为观察组55例和对照组54例,对照组实施小邮票皮片移植术、碳纤维敷料联合修复方案,观察 组实施Meek植皮技术、碳纤维敷料联合修复方案。比较两组手术相关指标(手术时间、皮片融合时间、创面愈合时间及住 院时间)、植皮情况(供皮面积、植皮面积率及术后2周皮片成活率),比较术前及术后12个月两组生活质量[简明烧伤健 康量表(Burn-specific health scale-abbreviated,BSHS-A)评分]、术后12个月瘢痕状况[温哥华瘢痕量表(Vancouver scar scale,VSS)评分],比较两组治疗12个月内并发症发生情况。结果:观察组手术时间、皮片融合时间、创面愈合时 间、住院时间均显著短于对照组,差异有统计学意义(P<0.05)。观察组供皮面积、植皮面积率显著低于对照组,差异有 统计学意义(P<0.05);观察组术后2周皮片成活率显著高于对照组,差异有统计学意义(P<0.05)。术后12个月,两 组BSHS-A评分均较治疗前显著升高,且观察组明显高于同一时间对照组,差异有统计学意义(P<0.05)。术后12个月观 察组VSS评分低于对照组,差异有统计学意义(P<0.05)。术后12个月,观察组并发症总发生率显著低于对照组,差异有 统计学意义(P<0.05)。结论:Meek植皮术联合碳纤维敷料可对大面积烧伤患者创面产生良好修复效果,有效缩短手术时 间及患者恢复时间,改善植皮情况并提高术后成活率,同时相关并发症发生较少,具有较高临床应用价值。
Abstract:
Objective To explore the effect of Meek skin grafting combined with carbon fiber dressing in repairing largearea burn wounds. Methods 109 patients with large-area burns who were treated in the hospital between January 2018 and January 2020 were randomly divided into the observation group (55 cases) and the control group (54 case). The control group underwent repair treatment regimen of small stamp skin grafting combined with carbon fi ber dressing, and the observation group implemented repair treatment regimen of Meek skin grafting technique combined with carbon fi ber dressing. The surgery related indicators (surgical time, skin fusion time, wound healing time and hospital stay) and skin graft status (skin donor area, skin graft area rate and skin graft survival rate at 2 weeks after surgery) were compared between the two groups. The quality of life (BSHS-A) of the two groups were compared before treatment and after 12 months of treatment. The scar status (VSS) of the two groups was compared after 12 months of treatment. The occurrence of complications within 12 months of treatment were compared. Results The surgical time, skin fusion time, wound healing time and hospital stay of the observation group were signifi cantly shorter than those of the control group, the diff erences were statistically signifi cant (P <0.05). And the skin donor area and skin graft area rate of the observation group were significantly lower than those of the control group (P<0.05). The skin graft survival rate at 2 weeks after surgery was significantly higher in the observation group than that in the control group (P<0.05). At 12 months after treatment, the BSHS-A scores of the two groups were significantly higher than those before treatment, and the score of the observation group was significantly higher than that of the control group at the same time point (P<0.05). The VSS score of the observation group was lower than that of the control group 12 months after operation (P<0.05). Within 12 months of treatment, the total incidence rate of complications in observation group was significantly lower than that in control group (P<0.05). Conclusion Meek skin grafting combined with carbon fi ber dressing can achieve a good repair eff ect on the wounds of patients with large-area burns, and effectively shorten the surgical time and recovery time of patients, improve the skin graft status and increase the postoperative survival rate. In addition, it has few complications and high clinical application value.
