摘要:
目的：探讨精准分层缝合联合壳聚糖医用生物凝胶对急诊面部外伤患儿伤口愈合的影响。方法：选取2021年1月-2022 年1月笔者医院整形外科急诊收治的面部外伤患儿90例，按随机数字表法分为对照组和联合组，各45例。两组患儿均给予精 准分层缝合，在此基础上，联合组每日换药后在伤口处涂抹壳聚糖医用生物凝胶。观察比较两组患儿伤口愈合情况、温哥华 瘢痕量表（Vancouver scar scale，VSS）评分、石溪瘢痕量表（Stony brook scar evaluation scale，SBSES）评分、美 观满意度评分及不良反应。结果：联合组患儿伤口愈合时间短于对照组（P＜0.05）；术后3个月，联合组患儿伤口愈合优良 率高于对照组，VSS评分低于对照组，SBSES评分、面部美观满意度评分高于对照组，以上指标组间比较差异均有统计学意义 （P＜0.05）；术后，联合组不良反应总发生率低于对照组（P＜0.05）。结论：采用精准分层缝合联合壳聚糖医用生物凝胶 治疗急诊面部外伤患儿，能促进患儿伤口愈合，减少瘢痕形成，提高美观满意度，且不良反应发生率低。 [
Abstract:
Objective The aim of this study was to investigate the effects of precise layered suture combined with chitosan medical biological gel on wound healing of children with emergency facial trauma. Methods A total of 90 children with facial trauma who were admitted to plastic surgery emergency department of the author’s hospital from January 2021 to January 2022 were selected,and assigned to a control group and a combination group by random number table method with 45 cases in each group.All children were given precise layered suture.On this basis,chitosan medical biological gel was applied to the wounds of the combination group after daily dressing changes.Wound healing,the vancouver scar scale (VSS) scores,the stony brook scar evaluation scale (SBSES) scores,aesthetic satisfaction and adverse reactions in the two groups were comparatively analyzed. Results The wound healing time of the combined group was shorter than that of the control group (P ＜0.05). At 3 months after operation,the excellent rate of wound healing of children in the combined group was higher than that in the control group,the VSS score was lower than that in the control group,and the SBSES score and facial aesthetic satisfaction score were higher than those in the control group,and the diff erences between the above indicators were statistically signifi cant (P ＜0.05). After operation,the total incidence of adverse reactions in the combined group was lower than that in the control group (P ＜0.05). Conclusion Applying precise layered suture combined with chitosan medical biological gel in the treatment of emergency facial trauma can promote wound healing,reduce scar formation,and improve aesthetic satisfaction,with a low incidence of adverse reactions.
