摘要:
目的:探讨颧牙槽嵴区微种植体支抗用于安氏Ⅱ类错牙合畸形的功能和美学效果。方法:选取2018年2月-2019年9月笔 者科室就诊的安氏Ⅱ类错牙合畸形患者96例,采用随机信封法分为研究组和对照组,每组48例。对照组予以高位头帽口外弓 治疗,研究组采用颧牙槽嵴区微种植体支抗治疗。治疗后比较两组患者骨组织指标、软组织指标、牙齿相关指标及美学效 果满意度。结果:上齿槽座角(SNA)、下颌角颏顶点连线与SN平面夹角(GoGn-SN)、下齿槽座角(SNB)治疗前后组间比较, 差异均无统计学意义(P>0.05);治疗后牙合平面与SN平面夹角(OPSN)较治疗前变小,研究组OPSN低于对照组(P<0.05);两 组患者治疗后下唇到E线距离(LLEP)、上唇到E线距离(ULEP)、上唇突度(ULP)均较治疗前降低,且研究组低于对照组 (P<0.05),Z角与治疗前相比变大,且研究组大于对照组(P<0.05);两组患者治疗后上中切牙长轴与SN平面夹角(U1- SN)、上中切牙切缘至NA连线距离(U1-NA)、翼上颌裂后缘至上颌磨牙远中邻面垂直距离(Ptm-U6)、Apg线至上中切牙切 缘距离(Apg-U1)均较治疗前降低,且研究组低于对照组(P<0.05);治疗后,两组患者面部美学效果均显著改善,且研 究组优于对照组,矫治时间研究组短于对照组,差异有统计学意义(P<0.05)。结论:颧牙槽嵴区微种植体支抗矫治安氏 Ⅱ类错牙合畸形,可更好地恢复咬合关系,更符合面部外观美学标准。
Abstract:
Objective To investigate the functional and aesthetic eff ects of zygomatic alveolar ridge microimplant anchorage for Class Ⅱ malocclusion. Methods Selected as 96 patients from February 2018 to September 2019 with class II malocclusion. They were divided into the study group and the control group by random envelope method, with 48 cases in each group. the control group received high-position headgear and oral for arch treatment, the research group used zygomatic alveolar ridge microimplant anchorage treatment. After treatment, the two groups of patients were compared with bone tissue indicators, soft tissue indicators, teeth-related indicators and satisfaction with cosmetic eff ects. Results The upper alveolar angle (SNA), the angle between the line of the chin vertex of the mandibular angle and the SN plane (GoGn-SN), and the lower alveolar angle (SNB) was no signifi cant diff erence between groups in treatment before and after(P >0.05). After treatment, the angle between the occlusal plane and the SN plane (OPSN) became smaller than before , and lower than that of the control group (P<0.05); the distance between the lower lip and the E line of the two groups of patients after treatment (LLEP), the distance from upper lip to E line (ULEP), and upper lip protrusion (ULP) were all lower than before, and lower than the control group (P<0.05), the Z angle became larger than before treatment, and the study group Greater than the control group (P<0.05). After treatment, the U1-SN, theU1-NA, the Ptm-U6 and the Apg-U1 were lower than before, and lower than the control group (P<0.05). After treatment, the facial cosmetic eff ects of the two groups of patients were signifi cantly improved, and the study group was higher than the control group, and the treatment time was shorter in the study group than the control group (P <0.05). Conclusion Compared with the treatment of Class Ⅱ malocclusion with high-position headgear and oral arch, the treatment of zygomatic alveolar ridge microimplant anchorage can achieve better clinical results, restore higher occlusal relationship, and more in line with the aesthetic standards of facial appearance.
