摘要:
目的：探讨隐形矫治器在矫治错牙合畸形伴牙周病过程中对患者牙周状况及相关炎性因子水平的影响。方法：收集2019年9月-2022年2月笔者医院正畸科就诊的符合纳入标准的66例患者，随机分为实验组（隐形矫治组）和对照组（固定矫治组）。比较两组在治疗过程中不同时间点的牙龈指数（Gingival index，GI）、龈沟出血指数（Sulcus bleeding index，SBI）、菌斑指数（Plaque index，PLI）、牙周探诊深度（Probing depth，PD）及龈沟液内白介素-1β（Interleukin-1β，IL-1β）和肿瘤坏死因子-α（Tumor necrosis factor-α，TNF-α）浓度水平的变化。结果：两组矫治后第1、3、6、9个月，GI、PLI、PD、SBI、IL-1β、TNF-α均高于矫治前，但差异无统计学意义（P＞0.05）；实验组在不同矫治阶段各牙周指标及相关炎性因子浓度均低于对照组（矫正后第1个月TNF-α浓度除外），差异有统计学意义（P＜0.05）。结论： 隐形矫治器更有利于轻中度错牙合畸形伴牙周病患者的牙周组织健康。
Abstract:
Objective To investigate the periodontal status and inflammatory factors of patients with malocclusion with periodontal disease treated by invisible appliance. Methods From September 2019 to February 2022, 66 patients in orthodontics department satisfying the inclusion criteria, were randomly divided into the experimental group(using invisible appliance) and control group(using fixed appliance）. The gingival index(GI),gingival crevicular bleeding index (BI), plaque index (PLI), periodontal probing depth(PD), and the concentration levels of interleukin (IL)-1β and tumor necrosis factor(TNF)-α in gingival crevicular fluid at different treatment time were compared between the two groups. Results GI, PLI, PD, SBI, IL -1β and TNF -α the experimental group and the control group were higher than those at the fi rst, third, sixth and ninth months after the treatment, but there was no statistical signifi cance (P ＞0.05).The concentrations of periodontal indexes and related infl ammatory factors in the experimental group at diff erent stages were lower than those in the control group (TNF-α concentration in the fi rst month of treatment was excluded), and the diff erence was statistically signifi cant (P ＜0.05). Conclusion Invisible appliance is more benefi cial to periodontal tissue health in patients with mild to moderate malocclusion with periodontal disease.
