摘要:
目的:探讨超脉冲点阵CO2激光联合重组人碱性成纤维细胞生长因子(rh-bFGF)治疗凹陷性痤疮瘢痕的疗效及对ECCA 评分的影响。方法:选择2019年8月-2020年8月笔者医院就诊的104例凹陷性痤疮瘢痕为研究对象,予以随机数字表法分对照 组(超脉冲点阵CO2激光)52例,观察组(超脉冲点阵CO2激光联合rh-bFGF凝胶)52例,比较两组患者临床疗效及治疗前、后 皮肤生理指标变化;记录两组患者红斑持续时间、结痂时间、痤疮瘢痕权重(ECCA)评分及不良反应情况。结果:对照组总 有效84.62%,观察组总有效率96.15%,对照组无效患者8例,观察组仅2例无效,差异有统计学意义(P<0.05)。观察组治 疗后ECCA评分显著低于对照组,观察组治疗后红斑持续时间、结痂时间短于对照组,差异有统计学意义(P<0.05)。观察 组术后角质层水含量高于对照组,TEWL、皮肤敏感性低于对照组,差异有统计学意义(P<0.05)。对照组不良反应发生率 17.30%,观察组不良反应发生率3.84%,差异有统计学意义(P<0.05)。结论:超脉冲点阵CO2激光联合rhbFGF治疗凹陷性 痤疮瘢痕效果明显,改善患者ECCA评分,不良反应较少,值得临床推广应用。
Abstract:
Objective To investigate the effect of ultra-pulse fractional CO2 laser combined with rh-bFGF in the treatment of depressed acne scar and its influence on ECCA score. Methods 104 cases of acne scar with depression from August 2019 to August 2020 were selected as the research object, A random number table was used to divide 52 cases into the control group (ultra-pulse fractional CO2 laser) and 52 cases for the observation group (ultra-pulse fractional CO2 laser combined with rhbFGF). The clinical efficacy and changes of skin physiological indexes before and after treatment were compared between the two groups; The duration of erythema, scab time, ECCA score and adverse reactions were recorded. Results The total eff ective rate of the control group was 84.62%, the total effective rate of the observation group was 96.15%. There were 8 invalid patients in the control group, and only 2 invalid patients in the observation group, the difference was statistically significant (P<0.05). The ECCA score of the observation group after treatment was significantly lower than that of the control group, the duration of erythema and scab time of the observation group after treatment were shorter than those of the control group, the difference was statistically signifi cant (P<0.05).After operation, the water content of stratum corneum in the observation group was higher than that in the control group, the TEWL and skin sensitivity in the observation group were lower than those in the control group, and the difference was statistically significant (P<0.05). The incidence of adverse reactions was 17.30% in the control group and 3.84% in the observation group, and the difference was statistically significant (P<0.05).Conclusion Ultra-pulse fractional CO2 laser combined with rh-bFGF has obvious effect in treating depressed acne scars. After treatment, the ECCAscore is significantly reduced, with fewer adverse reactions. It is worthy of clinical application.
