摘要:
目的：研究梅花针联合他克莫司软膏治疗头部脂溢性皮炎（Seborrheic dermatitis，SD）的有效性和安全性。方 法：选取于2019年1月-2021年1月在笔者医院接受治疗的168例头部SD患者，随机分为梅花针组和西药组，每组84例，西药组 给予他克莫司软膏治疗，梅花针组则在此基础上联合给予梅花针治疗，两组均治疗3个月后随访1年。比较两组临床疗效， 比较两组治疗前后的中医症状学积分、头皮状况[皮肤角质层含水量、油脂分泌、黏着性头皮鳞屑分值（Adherent scalp flaking scale，ASFS）]，比较随访1年内两组复发率及不良反应发生情况。结果：梅花针组治疗有效率98.81%高于西药组 （88.09%）（P＜0.05）。治疗3个月后，两组皮肤角质层含水量均较治疗前升高，中医症状学积分、油脂分泌和ASFS评分 均较治疗前减少（P＜0.05）；且梅花针组皮肤角质层含水量高于西药组，中医症状学积分、油脂分泌、ASFS评分低于西药 组，差异均有统计学意义（P＜0.05）；两组不良反应差异无统计学意义（P＞0.05）。随访1年，梅花针组复发率3.57%，低 于西药组（14.29%）（P＜0.05）。结论：梅花针联合他克莫司软膏可缓解SD患者临床症状，改善患者头皮状况，降低复发 风险，且两种治疗方式联合使用安全性尚可。 [
Abstract:
Objective To study the eff ectiveness and safety of plum-blossom needle combined with tacrolimus ointment in the treatment of seborrheic dermatitis (SD) on scalp. Methods 168 patients with SD on scalp who were treated in the hospital were selected between January 2019 and January 2021, and they were randomly divided into plum-blossom needle group and western medicine group, with 84 cases in each group. The western medicine group was given tacrolimus ointment for treatment while the plum-blossom needle group was treated with plum-blossom needle on this basis, and both groups were followed up for 1 year after 3 months of treatment. The clinical effi cacy, scores of TCM symptoms, scalp condition [water content of stratum corneum, grease secretion, Adherent Scalp Flaking Scale (ASFS)] before and after treatment and recurrence rate and occurrence of adverse reactions within 1 year of follow-up were compared between the two groups. Results The treatment eff ectiveness rate of 98.81% was higher in plum-blossom needle group than in the western medicine group (88.09%) (P ＜0.05). After 3 months of treatment, the water content of stratum corneum in the two groups was increased while the scores of TCM symptoms, grease secretion and ASFS score were decreased compared to before treatment (P＜0.05). The eff ective rate and water content of stratum corneum were higher while the scores of TCM symptoms, greases secretion, and ASFS score were lower in plum-blossom needle group compared with those in western medicine group at the same time point (P ＜0.05). There were no statistical diff erences in adverse reactions between the two groups (P ＞0.05). At 1-year follow-up, the recurrence rate of 3.57% in plum-blossom needle group was lower than that of the western medicine group (14.29%) (P＜0.05).Conclusion Plum-blossom needle combined with tacrolimus ointment can relieve the clinical symptoms of patients with SD, improve the scalp condition and reduce the risk of recurrence, and the safety of combined use of the two treatments is acceptable.
