摘要:
目的:探讨分析微种植体支抗联合正畸治疗成人双颌前突的临床效果。方法:选取2020年8月-2023年1月笔者医院就 诊的60例双颌前突患者,参照随机数字表法分为对照组(n=30)和研究组(n=30)。对照组予以传统支抗治疗,研究组予以 微种植体支抗联合正畸治疗,比较两组治疗前后软组织指标[上唇至审美平面的垂直距离(UI-E1)、下唇至审美平面垂直距 离(L1-E1)、上唇最低点与中切牙最高点距离(GSL)],硬组织指标[上中切牙唇倾角(UI-SN)、下中切牙唇倾角(L1-MP)、 上下中切牙长轴交角(UI-L1)、上中切牙切缘至腭平面垂直距离(U1/PP)、下切牙切缘至下颌平面垂直距离(L1/MP)],患者外 观满意度及并发症发生率。结果:治疗后,研究组L1-E1、UI-E1、GSL、L1-MP、U1/PP、UI-SN水平较对照组低,UI-L1、L1/MP 水平较对照组高(P<0.05);研究组并发症发生率13.33%,与对照组的6.67%比较差异无统计学意义(P>0.05);研究组外 观满意度93.33%,高于对照组的73.33%(P<0.05)。结论:微种植体支抗联合正畸治疗可有效内收成人双颌前突患者上下 前牙,从而改善成人双颌前突软硬组织侧貌,提高患者外观满意度,且安全性良好,宜临床推广应用。
Abstract:
Objective To explore and analyze the eff ect of micro-implant anchorage combined with orthodontics on bimaxillary protrusion in adults treated with micro-implant nail combined with orthodontics. Methods Sixty patients with bimaxillary protrusion in the author’s hospital from August 2020 to January 2023 were randomly divided into control group (n=30) and study group (n=30). The control group was treated with traditional anchorage, while the study group was treated with micro implant anchorage combined with orthodontic treatment. The soft tissue indexes [vertical distance from the upper lip to the aesthetic plane (UI-E1), vertical distance from the lower lip to the aesthetic plane (L1-E1), distance from the lowest point of the upper lip to the highest point of the central incisor (GSL)], hard tissue indexes [upper central incisor lip inclination (UISN), lower central incisor lip inclination (L1-MP), angle of intersection of upper and lower central incisors’ long axes (UI-L1), vertical distance from the upper central incisor incisor to the palate plane (U1/PP), vertical distance from the lower incisor to the mandibular plane (L1/MP)], patients’ appearance satisfaction and the incidence of complications were compared between the two groups before and after treatment. Results After treatment, the levels of L1-E1, UI-E1, GSL, L1-MP, U1/PP and UI SN in the study group were lower than those in the control group, while the levels of UI-L1 and L1/MP were higher than those in the control group (P<0.05). The incidence of complications in the study group was 13.33%, and and there was no diff erence compared with the control group 6.67%(P >0.05). The appearance satisfaction of the study group was 93.33% higher than that of the control group 73.33% (P<0.05). Conclusion Micro-implant anchorage combined with orthodontic treatment can eff ectively retract the upper and lower anterior teeth of adult bimaxillary protrusion patients, thus improving the profi le of soft and hard tissues of adult bimaxillary protrusion and improving the patient’s appearance satisfaction, and it is safe and suitable for clinical application.
