摘要:
目的：分析自体微小皮片植皮术在大面积深度烧伤创面中的修复效果。方法：选择2020年11月-2021年10月笔者医院收治的20例大面积深度烧伤患者作为研究对象。所有患者均接受自体微小皮片植皮修复，用电动取皮刀取厚约0.2 mm的自体皮片，剪成3 mm×3 mm微小皮片，以皮片间隔5 mm移植到烧伤创面。记录所有患者的植皮手术操作时间、皮片成活时间、创面愈合时间、供皮面积、皮片扩展倍数、皮片成活率、手术费用和不良反应情况。结果：本组20例患者，无1例死亡病例。20例患者的植皮手术操作时间为1.5～2.5 min/% TBSA，皮片成活时间7～10 d，创面愈合时间26～38 d。16例皮片基本全部成活，成活率80.00%(16/20)，无需再次植皮；4例残余创面较大与感染有关，行二期自体微小皮片植皮术后均康复出院。术后随访6个月，20例患者生活基本自理，对愈合的功能与形态均满意。结论：自体微小皮片移植修复大面积深度烧伤创面的皮片成活率高，无需特殊器械即可完成，易于在基层医院开展。同时也为皮源不足的严重烧伤患者提供了新的救治思路。
Abstract:
Objective To analyze the application of autologous micro-skin grafting in the repair of large area deep burn wounds. Methods 20 patients with large area deep burn wounds admitted to our hospital from November 2020 to October 2021 were selected as the research objects. All patients underwent autologous micro-skin grafting. Autologous skin with a thickness of 0.2 mm was taken by an electric dermatome, cutted into 3 mm×3 mm micro-skins and transplanted to the burn wounds at 5 mm intervals. The skin graft operation time, skin survival time, wound healing time, skin donor area, skin expansion multiple, skin survival rate, surgical cost and adverse reactions of all patients were recorded. Results There was no death in 20 patients in this group. The operation time of skin grafting in 20 patients was 1.5-2.5 min/% TBSA. The survival time of the skin was 7-10 d. The healing time of the wound was 26-38 d. The skins of 16 cases were survived, with a survival rate of 80.00% (16/20), and there was no need for skin grafting again. Four cases with large residual wounds were related to infection, and all of them recovered and were discharged after two-stage autologous micro skin grafting. All patients had been followed up for 6 months after operation, 20 patients basically self-cared themselves, and were satisfied with the function and shape of the healing. Conclusion Autologous micro-skin grafting for repairing large area deep burn wounds have a high survival rate of the microskin, which can be completed without special equipment, and is easy to carry out in primary hospitals. It also provides new treatment ideas for severe burn patients with insuffi cient skin sources.
