摘要:
目的:研究微种植体支抗辅助上颌磨牙整体远移矫治安氏Ⅱ类错牙合畸形伴轻中度拥挤病例的远期效果。方法:选取成 年正畸患者30例,根据治疗需要在双侧磨牙区各植入一枚微种植体支抗辅助推上颌磨牙远移。记录患者治疗前(T0)、治疗 结束时(T1)及治疗后3年(T2)的影像及模型资料。通过比较三个时间点患者头影测量值的差异,以观察微种植体支抗辅 助上颌磨牙整体远移的远期疗效。结果:平均治疗时间27.6个月,平均随访时间为36.3个月。治疗结束时,U6牙冠远移平均 值为4.4 mm,U1牙冠腭侧移动值为3.4 mm,牙冠向腭侧旋转3.7°,与治疗前相比差异均有统计学意义(P<0.05);治疗结 束后3年,U6在矢状向与轴倾角的变化与正畸治疗结束时比较差异均无统计学意义(P>0.05),但∠MP-FH与PHF/AFH的变化 具有统计学意义(P<0.05)。结论:微种植体支抗辅助上颌磨牙向远中移动时,磨牙移动基本为冠根整体移动,并且远期 疗效稳定。
Abstract:
Objective To investigate long-term treatment stability of mini-implants anchored maxillary distalization in Class Ⅱ malocclusion. Methods This retrospective study included 30 adults with Class Ⅱ malocclusion. Mini-implants was used to maxillary distalization. Lateral cephalograms of all patients were taken before (T0), immediately after (T1), and 3 years after treatment (T2). Lateral cephalograms analysis, horizontal and vertical movement and tipping of the maxillary fi rst molars (U6) and central incisors (U1) were measured at 3 time points to compare the achieved treatment eff ects and long-term stability. Results The average duration of treatment was 27.6 months. The average follow-up time was 36.3 months. At the end of treatment, the U6 dental crown average distance value was 4.4 mm,the U1 palatal side of the tooth crown average distance value was3.4 mm, the crown rotates to the palatal side 3.7°.Compared with before treatment, both of them has statistically signifi cant (P<0.05). 3 years after the end of treatment, the variation of sagittal direction and axial dip angle of U6 compaired with T1 have no statistical differences (P>0.05). But the variation of ∠MP-FH and PHF/AFH has statistically significant(P<0.05). Conclusion In Class Ⅱ treatment, mini-implants anchored maxillary upper arch distalization can provide stable distal movement of the maxillary fi rst molars and central incisors.
