摘要:
目的：探讨口腔种植患者应用拔牙位点保存术的效果及对不同骨缺损类型的影响。方法：选取2017年6月-2020年6月 于笔者医院行种植修复的患者58例。按照随机数字表法分为常规组和研究组，每组29例，按拔牙后不同骨缺损类型将两组进 一步分组为一壁、二壁、三壁、四壁。常规组使用常规拔牙种植术，研究组使用拔牙位点保存术种植修复。术后6个月，观 察两组种植成功率、美观度及满意度。术后即刻、术后6个月，观察两组牙槽骨骨高度和宽度，并计算术后6个月平均骨恢复 率。 结果：研究组种植成功率100.00%，高于常规组的86.21%（P＜0.05）；研究组美观度、满意度评分均高于常规组，差 异有统计学意义（P＜0.05）。研究组的平均骨高度均增加，一、二壁增量大于三、四壁，常规组三、四壁的平均骨高度增 加，而一、二壁减小，且研究组骨高度变化均大于常规组，差异均有统计学意义（P＜0.05）。研究组一、二、三壁的平均 骨宽度均增加，四壁的平均骨宽度减小，一、二、三壁增量大于四壁；常规组的平均骨宽度均减小，一、二壁减小量大于 三、四壁，且研究组骨宽度变化均大于常规组，差异有统计学意义（P＜0.05）。研究组一、二壁的平均骨恢复率均高于常 规组（P＜0.05），两组三、四壁的平均骨恢复率比较差异无统计学意义（P＞0.05）。结论：拔牙位点保存术能提高口腔种 植患者的种植成功率，缓解骨吸收，具有较高的美观度、满意度，且骨缺损越小，骨量恢复效果越好。
Abstract:
Objective To investigate the effect of extraction site preservation in oral implant patients and its influence on diff erent types of bone defects. Methods Total of 58 patients who underwent implant restoration in the author’s hospital from June 2017 to June 2020 were selected. According to random number table method, the patients were divided into conventional group and research group, with 29 cases in each group. According to diff erent types of bone defects after extraction, the two groups were further divided into one wall, two wall, three wall, four wall. The routine group was treated with conventional extraction and implantation, and the study group was treated with extraction site preservation method for implant restoration. 6 months after surgery, the success rate of implantation, aesthetic degree and satisfaction degree of the two groups were observed. Immediately after surgery and 6 months after surgery, the height and width of alveolar bone of the two groups were observed, and the average bone recovery rate was calculated at 6 months after surgery. Results The success rate of implantation in the study group 100.00% was higher than that in the routine group 86.21% (P＜0.05). The aesthetics and satisfaction scores of the study group were higher than those of the routine group,the diff erences were statistically signifi cant(P ＜0.05). The average bone height of the study group increased, the increment of the one and two wall were greater than the three and four wall, the average bone height of the three and four wall in the routine group increased, while the one and two wall decreased, and the changes of the bone height in the study group were greater than the routine group,the diff erences was statistically signifi cant(P ＜0.05). The average bone width of the one and two and three wall of the study group increased, the average bone width of the four wall decreased, the increment of the one and two and three wall were greater than the four wall, the average bone width of the routine group decreased, and the reduction of the one and two wall were greater than the three and four wall, and the changes of the bone width in the study group were greater than the routine group (P＜0.05). The mean bone recovery rate in both the fi rst and second walls of the study group was higher than that of the conventional group (P ＜0.05), while there was no signifi cant difference in the mean bone recovery rate between the two groups in the third and fourth walls (P ＞0.05). Conclusion Extraction site preservation can improve the implant success rate of oral implant patients, alleviate bone resorption, has high aesthetics and satisfaction, and the smaller the bone defect, the better the bone volume recovery.
