摘要:
目的：探究脂质水胶体硫酸银敷料结合负压封闭引流治疗难愈性创面的临床疗效。方法：选取2022年2月-2022年12月 来笔者医院就诊的80例慢性难愈性创面患者，按随机单盲分组法分为实验组和对照组，每组40例。对照组采用负压封闭引流 技术进行治疗，实验组采用硫酸银敷料联合负压封闭引流技术治疗。观察比较两组创面愈合情况、炎症指标水平、创面疼痛 程度及患者睡眠质量。结果：两次更换敷料时，实验组细菌清除率均高于对照组（P＜0.05）；治疗2周后，实验组创面愈合 率高于对照组，肉芽生长评分高于对照组，创面疼痛评分低于对照组（P＜0.05）；治疗2周后，实验组血清白细胞、C-反应 蛋白、红细胞沉降率及创面组织中肿瘤坏死因子-α、白细胞介素-6水平均低于对照组（P＜0.05）。两组治疗前及治疗2周 后PSQI评分比较，差异均无统计学意义（P＞0.05）。结论：脂质水胶体硫酸银敷料结合负压封闭引流治疗慢性难愈性创面 能抑制细菌繁殖，降低感染风险，促进创面愈合，效果显著。
Abstract:
Objective To explore the clinical effi cacy of lipid hydrocolloid silver sulfate dressing combined with vacuum sealing drainage in the treatment of refractory wounds. Methods Eighty patients with chronic refractory wounds who came to author’s hospital from February 2022 to December 2022 were selected and divided into the experimental group and the control group according to the random single-blind grouping method, with 40 cases in each group. The control group was treated with vacuum sealing drainage technology, and the experimental group was treated with silver sulfate dressing combined with vacuum sealing drainage technology. The wound healing, inflammatory index level, wound pain degree and sleep quality of patients were observed and compared between the two groups. Results When dressing was changed twice, the bacterial clearance rate of the experimental group was higher than that of the control group (P＜0.05). After 2 weeks of treatment, the wound healing rate and granulation growth score of the experimental group were higher than those of the control group, and the wound pain score was lower than that of the control group (P＜0.05). After 2 weeks of treatment, the serum white blood cells, C-reactive protein, and erythrocyte sedimentation rate, and the levels of tumor necrosis factor-α and interleukin-6 in wound tissue in the experimental group were lower than those in the control group (P＜0.05). There was no statistically signifi cant diff erence in PSQI scores between the two groups before treatment and 2 weeks after treatment (P ＞0.05). Conclusion Lipid hydrocolloid silver sulfate dressing combined with vacuum sealing drainage in the treatment of chronic refractory wounds can inhibit bacterial reproduction, reduce the risk of infection, and promote wound healing, with signifi cant eff ect.
