摘要:
目的：分析富血小板血浆（Platelet-rich plasma,PRP）联合曲安奈德局部注射及瘢痕组织瓣修复术治疗瘢痕疙瘩的 美学效果。方法：选取2017年2月-2021年11月遂宁市中心医院医疗美容科收治的瘢痕疙瘩患者85例，遵随机数字表法分为曲 安奈德组42例（瘢痕组织瓣修复术联合曲安奈德局部注射）和联用PRP组43例（曲安奈德组基础上联合PRP治疗）。比较两组临 床疗效，瘢痕恢复情况，疼痛及瘙痒情况和复发率，记录两组不良反应发生情况。结果：治疗后，联用PRP组治疗总有效率为 97.67% ，显著高于曲安奈德组的85.71%（P＜0.05）；治疗后，两组色泽、柔软度、血管及厚度评分均降低（P＜0.05），且联 用PRP组显著低于曲安奈德组（P＜0.05）；治疗后，两组疼痛及瘙痒评分均降低（P＜0.05），且联用PRP组显著低于曲安奈 德组（P＜0.05）；两组不良反应发生率差异无统计学意义（P＞0.05）；随访1年后，联用PRP组总复发率为2.33%，显著低 于曲安奈德组的14.29%（P＜0.05）。结论：PRP联合曲安奈德局部注射联合瘢痕组织瓣修复术治疗瘢痕疙瘩疗效确切，能有 效促进瘢痕创面恢复，缓解疼痛及瘙痒，降低复发率。
Abstract:
Objective To analyze the cosmetic effect of platelet-rich plasma (PRP) combined with topical injection of triamcinolone and scar tissue fl ap repair in the treatment of keloids. Methods 85 patients with keloid treated in department of medical cosmetology of Suining Central Hospital from February 2017 to November 2021 were selected and divided into 42 cases in triamcinolone group (scar tissue fl ap repair combined with topical injection of triamcinolone) and 43 cases in united PRP group (PRP on the basis of triamcinolone group) according to the random number table method. The clinical effi cacy, scar recovery, pain and pruritus and recurrence rate were compared between the two groups, and the adverse reactions were recorded in the two groups. Results After treatment, the total eff ective rate of treatment in united PRP group was 97.67%, which was signifi cantly higher than 85.71% in triamcinolone group (P ＜0.05). The scores of color, softness, blood vessel and thickness were decreased in both groups after treatment (P＜0.05), and the scores were signifi cantly lower in united PRP group than those in triamcinolone group (P ＜0.05). After treatment, the scores of pain and pruritus were reduced in the two groups (P＜0.05), and the scores in united PRP group were signifi cantly lower compared to triamcinolone group (P ＜0.05). There were no statistically signifi cant diff erences in the incidence rates of adverse reactions between the two groups (P ＞0.05). After 1 year of follow-up, the total recurrence rate in united PRP group was 2.33%, which was signifi cantly lower than 14.29% in triamcinolone group (P ＜0.05). Conclusion PRP combined with topical injection of triamcinolone and scar tissue fl ap repair in the treatment of keloids has an exact effi cacy, and it can eff ectively promote the recovery of scar wound, relieve the pain and pruritus, and reduce the recurrence rate.
