摘要:
目的：比较普萘洛尔每日相同剂量不同给药频次治疗婴幼儿血管瘤的疗效，为临床治疗提供安全方便有效的给药次 数。方法：将2016年10月-2017年2月就诊于临沂市人民医院且符合入选标准的37例婴幼儿血管瘤患儿进行口服普萘洛尔治 疗，每日剂量2 mg/kg，根据给药次数的不同，采用随机数字表法分为A、B两组。A组19例，每日给药2次；B组18例，每日 给药3次。两组时间间隔均为8 h。记录治疗过程中不良反应发生情况、有效病例和治愈病例。6个月时对两组有效率、治愈 率及不良反应发生率进行比较。结果：37例婴幼儿血管瘤患儿在口服普萘洛尔后1周内，可见瘤体张力下降，颜色变淡。 治疗6个月时，A组19例患儿疗效评价Ⅰ级（差）1例，Ⅱ级（中）0例，Ⅲ级（好）4例，Ⅳ级（优）14例，有效率94.7% （18/19），治愈率73.7%（14/19）；不良反应发生率10.5%（2/19）。B组18例患儿疗效评价Ⅰ级（差）0例，Ⅱ级（中）0 例，Ⅲ级（好）4例，Ⅳ级（优）13例，因不良反应改变治疗剂量1例，有效率94.4%（17/18），治愈率72.2%（13/18）；不 良反应发生率11.1%（2/18）。6个月时两组有效率、治愈率、不良反应发生率比较差异无统计学意义（P＞0.05）。结论： 口服普萘洛尔每日2 mg/kg的剂量分2次给药与3次给药均安全、有效。推荐用药次数为每日2次。
Abstract:
Objective To study the efficacy of different administration frequencies of the same dose propranolol for infant hemangiomas , so as to provide safe, convenient and effective method. Methods From October 2016 to February 2017 ， there were 37 infants in accordance with the inclusion criteria,who were treated with propranolol at a dose of 2 mg/kg per day. According to the diff erent administration frequencies, they were randomly divided into A and B groups. Group A, 19 cases,a BID dosing; Group B, 18 cases, a TID dosing. The administration interval was eight hours . The eff ective rate, cure rate and adverse effect rate were compared at six months after treatment. Results 37 cases of infantile hemangiomas were treated. Primary effi cacy was assessed at one week after treatment. The tumors were smaller. The color changed from bright red to purplish red. At 6 months after treatment ,the curative eff ects were evaluated as follows: The treatment group A (19 cases): grade I (poor) 1 case, grade Ⅱ (fair) 0 case, grade Ⅲ (good) 4 cases, grade Ⅳ (excellent) 14 cases. The effective rate was 94.7 %(18/19).The cure rate was 73.7%(14/19). The incidence of adverse reactions was 10.5%(2/19).The treatment group B(18 cases): grade I (poor) 0 case, grade Ⅱ (fair) 0 case, grade Ⅲ (good) 4 cases, grade Ⅳ (excellent) 13 cases. The eff ective rate was 94.4 %(17/18).The cure rate was 72.2%(13/18). The incidence of adverse reactions was 11.1%(2/18).The eff ective rate and the cure rate were not statistically signifi cant(P ＞0.05).The incidence of adverse eff ects was not statistically signifi cant(P ＞0.05). Conclusion The administration frequencies of oral propranolol at 2mg/kg/d in infantile hemangiomas both 3 times per day and 2 times per day are safe and eff ective. The administration frequency of 2 times per day is recommended.
