摘要:
目的:探究纳米银抗菌水凝胶敷料联合重组碱性成纤维细胞生长因子(Recombinant human basic fibroblast growth factor,rh-bFGF)对踝关节开放性骨折术后恢复的影响。方法:选取2020年1月-2022年12月于庐江县中医院接受创 面修复的97例踝关节开放性骨折患者,按随机数字表法将其分为对照组(n=48例)和观察组(n=49例)。两组均行关节复位 固定术,对照组术后予以凡士林敷料进行创面修复,观察组患者予以纳米银抗菌水凝胶敷料联合rh-bFGF进行创面修复。比 较两组患者创面修复效果、关节疼痛肿胀情况、血清炎症因子水平[白介素-6(IL-6)、C-反应蛋白(CRP)、肿瘤坏死因子 α (TNF-α)]及创面并发症发生情况。结果:观察组治疗总有效率高于对照组(P<0.05),创面血运恢复时间与愈合时间 短于对照组(P<0.05);治疗8周后,观察组VAS评分、关节肿胀评分均低于对照组(P<0.05);治疗期间,观察组并发症 发生率低于对照组(P<0.05)。结论:纳米银抗菌水凝胶敷料联合rh-bFGF利于踝关节开放性骨折患者术后创面及关节恢 复,值得临床推荐。
Abstract:
Objective To explore the eff ect of nano-silver antibacterial hydrogel dressing combined with recombinant human basic fi broblast growth factor (rh- bFGF) on on the postoperative recovery of ankle joint open fracture. Methods A total of 97 patients with open ankle fractures who underwent wound repair at Lujiang County Hospital of Traditional Chinese Medicine from January 2020 to December 2022 were divided into a control group (n=48) and an observation group (n=49) by random number table method. Patients in the control group were treated with vaseline dressing for wound repair, while those in the observation group were treated with nano-silver antibacterial hydrogel dressing combined with rh- bFGF for wound repair. The eff ect of wound repair, joint pain and swelling, serum infl ammatory factors (interleukin-6 (IL-6), C-reactive protein (CRP), tumor necrosis factor α (TNF-α)) and the incidence of wound complications were compared between the two groups. Results The total eff ective rate of the observation group was higher than that of the control group (P <0.05), and the wound blood fl ow recovery time and healing time were shorter than that of the control group (P <0.05). After 8 weeks of treatment, VAS score and joint swelling score of observation group were lower than those of control group (P<0.05). During treatment, the complication rate of observation group was lower than that of control group (P<0.05). Conclusion Nano silver antibacterial hydrogel dressing combined with rh-bFGF is benefi cial to postoperative wound recovery and joint recovery in patients with ankle joint open fracture, which is worthy of clinical recommendation.
