摘要:
目的：探讨慢性牙周炎(Chronic periodontitis,CP)患者病情对口腔健康相关生活质量（Oral health-related quality of life,OHRQoL）的影响。方法：选择笔者医院2019年5月-2021年10月收治的108例CP患者为研究对象，按照病情 严重程度将患者分为轻度组（n=50）、中度组（n=42）和重度组（n=16）。对比三组患者一般资料、OHRQoL评分、美观满 意度以及病情相关指标。采用Pearson相关模型分析CP病情严重程度与美观满意度和OHRQoL的相关性；采用多因素线性回 归分析影响CP患者OHRQoL的因素。结果：总共发放108份问卷，剔除无效问卷11份，最终回收有效问卷97份，问卷回收率 97/108 （89.81%），最终纳入患者轻度组45例，中度组38例，重度组14例。三组患者生活质量评分对比差异均具有统计学 意义（P＜0.05），轻度组低于中度组和重度组，中度组低于重度组（P＜0.05）；三组美观满意度对比差异均具有统计学意 义（P＜0.05），轻度组高于中度组和重度组，中度组高于重度组（P＜0.05）；三组牙周相关指标对比差异均具有统计学意 义（P＜0.05），轻度组各指标低于中度组和重度组，中度组各指标低于重度组（P＜0.05）；CP患者OHRQoL评分与美观满意 度呈负相关(r=-0.721，P＜0.001)；OHRQoL评分与BI、PD、CAL以及PLI呈正相关（r=0.621、r=0.776、r=0.438、r=0.565， 均P＜0.001）；美观满意度与BI、PD、CAL以及PLI呈负相关（r=0.386、r=0.612、r=0.322、r=0.566，均P＜0.001） 多因 素线性回归分析结果显示：美观满意度以及疾病相关指标中BI、PD、CAL是影响患者OHRQoL的相关因素，标准化系数分别为 （-0.295、0.248、0.404、0.118，均P＜0.05）。结论：BI、PD、CAL以及美观满意度均对CP患者的OHRQoL产生影响。
Abstract:
Objective To explore the infl uence of chronic periodontitis (CP) on oral health-related quality of life ( OHRQoL). Methods A total of 108 patients with CP treated in author’s hospital from May 2019 to October 2021 were taken as the research objects, and they were divided into mild group (n=50), moderate group (n=42) and severe group (n=16) according to their severity. The general information, OHRQoL score, aesthetic satisfaction and related indexes of the three groups were compared. Pearson correlation model was used to analyze the correlation between CP severity and aesthetic satisfaction and OHRQoL. Multivariate linear regression analysis was used to analyze the influencing factors of OHRQoL in CP patients. Results A total of 108 questionnaires were distributed, 11 invalid questionnaires were rejected, and 97 valid questionnaires were finally collected, with a questionnaire recovery rate of 97/108(89.81%), of which 5 were rejected in mild group, 4 in moderate group and 2 in severe group. Finally, 45 patients were included in mild group, 38 in moderate group and 14 in severe group. There were statistically signifi cant diff erences in quality of life scores among the three groups (P ＜0.05). The mild group was lower than the moderate and severe groups, and the moderate group was lower than the severe group (P＜0.05). There were signifi cant diff erences in aesthetic satisfaction among the three groups (P ＜0.05). The mild group was higher than the moderate group and the severe group, and the moderate group was higher than the severe group (P＜0.05). There were statistically signifi cant diff erences in periodontal indexes among the three groups (P ＜0.05). The indexes of mild group were lower than those of moderate group and severe group, and those of moderate group were lower than those of severe group (P＜0.05). There was a negative correlation between CP OHRQoL score and aesthetic satisfaction (r=-0.721, P ＜0.001). OHRQoL score was positively correlated with BI, PD, CAL and PLI (r=0.621, r=0.776, r=0.438, r=0.565, all P ＜0.001). Aesthetic satisfaction was negatively correlated with BI, PD, CAL and PLI (r=0.386, r=0.612, r=0.322, r=0.569, all P＜0.001). The results of multivariate linear regression analysis showed that BI, PD and CAL were the related factors aff ecting patients’ OHRQoL among aesthetic satisfaction and disease-related indicators, and the standardized coeffi cients were respectively. Conclusion BI, PD, CAL and aesthetic satisfaction can infl uence OHRQoL of CP patients.
