摘要:
目的：系统评价针刺治疗黄褐斑的临床疗效，为临床针刺疗法对黄褐斑的治疗应用提供可靠的循证依据。方法：计算机 检索PubMed、Cochrane图书馆、Web of science、Embase、中国知网、中国科学期刊数据库和万方数据库。搜集自建库至2021 年5月发表的有关针刺治疗黄褐斑的随机对照试验（Randomized controlled trial，RCT），对符合纳入标准的文献进行资料提 取和质量评价，应用RevMan 5.3 统计软件进行Meta分析。结果：最终纳入15篇RCTs，共计1 429例黄褐斑患者。Meta分析结果显 示针刺治疗组在提高总有效率[OR=3.68，95%CI（2.65，5.11），Z=7.78，P＜0.000 01]方面优于对照组（西药、中药、物理治 疗）；针刺治疗组在提高黄褐斑皮损面积评分[MD=-0.19，95%CI（-0.23，-0.15），Z=9.40，P＜0.000 01]，黄褐斑皮损颜色 评分[MD=-0.47，95%CI（-0.52，-0.42），Z=18.64，P＜0.000 01]方面，均优于对照组（西药治疗、中药治疗）。且只有5项 研究提到了不良反应。结论：本研究对针刺治疗黄褐斑的RCT试验进行Meta分析，证明临床针刺治疗对黄褐斑疗效确切。
Abstract:
Objective To systematically evaluate the clinical effi cacy of acupuncture in the treatment of melasma, and to provide a reliable evidence-based basis for the clinical application of acupuncture in the treatment of melasma. Methods PubMed, Cochrane Library, Web of Science, Embase, China National Knowledge Infrastructure (CNKI), China Science Journal Database (VIP) and Wanfang Database were searched by computer. Randomized controlled trials (RCTS) on acupuncture treatment of melasma published from the self-established database to May 2021 were collected. Data extraction and quality evaluation were performed for the literatures that met the inclusion criteria. Meta-analysis was performed using RevMan 5.3 statistical software. Results Finally, 15 RCTs were included, with a total of 1 429 patients with melasma. The results of meta-analysis showed that:Acupuncture treatment group was better than control group (western medicine, traditional Chinese medicine, physical therapy) in improving the total effective rate [OR=3.68 ，95%CI(2.65,5.11)，Z=7.78,P＜0.000 01]; In the acupuncture treatment group, the area score of melasma skin lesions [MD=-0.19,95%CI（-0.23,-0.15）,Z=9.40,P＜0.000 01] and color score of melasma skin lesions [MD=-0.47,95%CI（-0.52,-0.42），Z=18.64，P＜0.000 01], were better than the control group (western medicine, traditional Chinese medicine). Only 5 studies mentioned adverse reactions. Conclusion This study conducted a meta-analysis of the RCT trials of acupuncture in the treatment of melasma, and proved that clinical acupuncture has a defi nite eff ect on melasma, indicating that acupuncture is eff ective in the treatment of melasma.
