摘要:
目的：观察消斑方联合氨甲环酸口服对血管型黄褐斑的临床疗效及其在反射式共聚焦显微镜（Reflectance confocal micro scopy,RCM）下的影像学变化。方法：将2021年1月-2022年6月笔者医院收治的60例血管型黄褐斑患者（肝肾阴虚证）随 机分为两组，对照组（n =30）予以氨甲环酸片口服；治疗组（n =30）在对照组的基础上予以消斑方治疗。12周后进行 临床疗效评定及RCM监测其影像学的变化。结果：治疗12周后，两组疗效总体差异有统计学意义，治疗组疗效优于对照组 （P＜0.05）；RCM监测下，两组患者树突状细胞形态改变，炎症细胞、黑素细胞、血管数量减少，且治疗组改善优于对照组 （P＜0.05）；随访2个月后，对照组复发率为33.33%高于治疗组复发率为7.14%（P＜0.05）。结论：使用消斑方联合氨甲环酸 治疗血管型黄褐斑的临床疗效优于单纯口服氨甲环酸，且复发率明显降低。运用RCM能很好地评估其临床疗效。
Abstract:
Objective To observe the clinical eff ect of Xiaoban Decoction combined with tranexamic acid on vascular chloasma and its imaging changes under RCM. Methods 60 patients with vascular chloasma (liver and kidney yin defi ciency syndrome) admitted to the author’s hospital from January 2021 to June 2022 were randomly divided into two groups. The control group (30 cases) was given tranexamic acid tablets orally.The treatment group (30 cases) was treated with Xiaoban Decoction on the basis of the control group. After 12 weeks, the clinical effi cacy was evaluated and the imaging changes were monitored by RCM. Results After 12 weeks of treatment, The overall diff erence in curative eff ect between the two groups was statistically signifi cant, and the effi cacy of the treatment group was better than that of the control group(P ＜0.05). Under RCM monitoring, the morphology of dendritic cells changed, the number of infl ammatory cells decreased, the number of melanocytes decreased and the number of blood vessels decreased in the two groups, and the improvement in the treatment group was better than that in the control group (P＜0.05).After 2 months’ follow-up, the recurrence rate in the control group was 33.33%, which was higher than that in the treatment group (7.14%)(P＜0.05). Conclusion The clinical eff ect of Xiaoban Decoction combined with tranexamic acid in treating vascular chloasma is better than that of tranexamic acid alone, and the recurrence rate is obviously reduced. RCM can be used to evaluate its clinical effi cacy.
