摘要:
目的:探讨超高频超声对糖皮质激素依赖性皮炎的定量检测效果,并分析其在皮肤屏障功能评价中的评估价值。方 法:选取2021年1月-2022年9月于笔者医院接受治疗的21例糖皮质激素依赖性皮炎患者作为研究对象纳入研究组,同时选取 同期于笔者医院接受体检的23例健康志愿者作为对照组。应用超高频超声(频率22 MHz)测量患者面部皮肤的表皮厚度、 真皮浅层厚度、真皮深层厚度及真皮血供等指标,并通过皮肤屏障功能评价指标经皮水分丢失(Transepidermal water loss,TEWL)评分评价超高频超声的评估价值。结果:多因素Logistic分析显示,TEWL评分(OR=1.833,P=0.045)与彩色多 普勒血流成像(Color Doppler flow imaging,CDFI)(OR=1.606,P=0.047)是糖皮质激素依赖性皮炎发生的独立影响因 素;ROC曲线分析显示CDFI指标(AUC=0.853,特异性=91.3%,敏感性=76.2%)与TEWL评分(AUC=0.851,特异性=95.7%,敏 感性=66.7%)的评估价值相当。结论:超高频超声可对糖皮质激素依赖性皮炎患者的面部皮肤进行定量检测,且将其用于皮 肤屏障功能评价中也同TEWL评分一样具有相同的评估价值。
Abstract:
Objective To investigate the eff ect of ultra-high-frequency ultrasound in the quantitative detection of corticosteroid addictive dermatitis and analyze its diagnostic value in the evaluation of skin barrier function. Methods The 21 patients with corticosteroid addictive dermatitis who were treated in the author’s hospital from January 2021 to September 2022were selected as the research subjects to be included in the research group, and 23 healthy volunteers who were subjected to physical examination in the author’s hospital during the same period were selected as the control group.Ultra-high frequency ultrasound (22 MHz) was used to measure the epidermal thickness, superfi cial dermal thickness, deep dermal thickness and dermal blood supply of patients’ facial skin, and the evaluation value of ultra-high frequency ultrasound was evaluated through the evaluation index of skin barrier function, transepidermal water loss (TEWL) score. Results Multivariate Logistic analysis showed that TEWL score (OR=1.833, P=0.045) and color Doppler fl ow imaging (CDFI) (OR=1.606, P=0.047) were independent infl uencing factors for the occurrence of corticosteroid addictive dermatitis.ROC curve analysis shows that the evaluation value of CDFI index(AUC=0.853, Specifi city =91.3%, and Sensitivity =76.2%) is equivalent to TEWL score(AUC=0.851, Specifi city =95.7%, and Sensitivity =66.7%). Conclusion Ultra-high frequency ultrasound can quantitatively detect the facial skin of patients with corticosteroid addictive dermatitis, and it has the same evaluation value as TEWL score when it is used to evaluate the skin barrier function.
