摘要:
目的:探讨第一磨牙严重龋坏或者缺失后,通过正畸前移第二磨牙关闭第一磨牙缺隙的临床疗效。方法:采用直丝 弓矫治技术,前移第二磨牙关闭由第一磨牙严重龋坏拔除或缺失造成缺隙的错牙合畸形患者22例,通过在曲面断层片上测量 第二前磨牙与第二磨牙牙体长轴成角评价牙根平行情况,并观察第二磨牙牙根吸收和牙槽骨改建情况。结果:第二磨牙 全部前移并关闭第一磨牙缺隙,第三磨牙排齐或待萌出,患者牙列咬合关系良好,软组织侧貌协调。矫治完成时间平均 (24.8±4.3)个月。治疗后,第二前磨牙牙根基本平行于第二磨牙牙根,两牙齿牙体长轴成角减小,差异具有统计学意义 (P<0.05)。结论:第一磨牙病损或者缺失的错牙合畸形,第三磨牙(或牙胚)存在时,正畸前移第二磨牙关闭第一磨牙间 隙是临床可行的治疗方法。
Abstract:
Objective To investigate the clinical eff ect of the orthodontic treatment which have severe caries or loss of the fi rst molar, by forward the second molar to close the missing gap of the fi rst molar. Methods 22 cases of malocclusion with severe caries or loss of the fi rst molar were treated with straight wire appliance. The root parallelism was evaluated by measuring the angulations between the long axis of the second premolar and the second molar on panoramic radiograph, and the root resorption and alveolar bone remodeling of the second molar were observed. Results The second molars were successfully replaced first molar position, patients get nice occlusion, normal overbite and overjet, soft tissue profile coordinate. The average treatment time was (24.8±4.3) months. The roots of the second premolars and the second molarswere basically parallel after treatment, and the long axis angulationsof the two teeth decreased, the diff erence was statistically signifi cant (P <0.05). Conclusion Mesial movement of second molar treatment is clinically feasible when malocclusion patients with lesion or deletion of the fi rst molar have the third molar or dental germ.
