摘要:
目的:探讨牙周炎患者行牙种植同期引导骨组织再生术(Guided bone regeneration,GBR)的效果以及对骨改 建、牙周状态的影响。方法:选取2019年6月-2021年6月笔者医院收治的符合纳入标准的64例牙周炎伴牙体缺损患者为研 究对象,根据患者牙周骨量情况分为两组,各32例。对照组患者行单纯牙种植手术治疗,观察组患者行牙种植同期GBR术 治疗。治疗后1年复查,比较两组种植体脱落率、松动度及附着龈状态等情况,比较两组治疗前后牙菌斑指数(Plaque index,PLI)、牙周探诊深度(Probing depth,PD)及牙龈指数(Gingival index,GI)等牙周状态指标,比较两组术后3个 月及1年时颊舌侧骨吸收和近远中骨吸收情况,比较两组白细胞介素(Interleukin,IL)-1β、肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)及C-反应蛋白(C-reactive protein,CRP)等炎症因子水平。结果:治疗后,两组种植体脱 落率、存留率、松动度及附着龈宽度比较差异无统计学意义(P>0.05);治疗后,两组PLI、PD、GI较治疗前明显下降,且 观察组PD低于对照组(P<0.05),两组PLI、GI比较差异无统计学意义(P>0.05);术后3个月及术后1年,观察组颊舌侧 骨吸收量、近远中骨吸收量均低于对照组(P<0.05);治疗后,两组IL-1β、TNF-α、CRP等炎症因子水平较治疗前明显下 降,且观察组低于对照组(P<0.05)。结论:采用牙种植同期GBR治疗牙周炎伴牙体缺损患者疗效显著,可短期内改善种植 体边缘骨吸收情况,维持患者牙周状态稳定,减轻牙周组织炎症反应。
Abstract:
Objective To investigate the eff ects of simultaneous guided bone regeneration (GBR) with dental implants on bone remodeling and periodontal status in patients with periodontitis. Methods From June 2019 to June 2021, 64 periodontitis with tooth defect were selected as the research objects. According to the periodontal bone mass, the patients were divided into two groups, 32 cases in each group. The patients in the control group were treated with simple dental implant surgery, while the patients in the observation group were treated with dental implant and GBR at the same time. One year after the treatment, the two groups were compared in terms of implant loosening rate, implant mobility, gum attachment status, plaque index (PLI), probe depth (PD) and gingival index (GI). The buccolingual bone resorption and mesiodistal bone resorption were compared between the two groups at 3 months and 1 year after operation, and the levels of infl ammatory factors such as interleukin- 1β (IL-1β), tumor necrosis factor-α (TNF-α) and C-reactive protein (CRP) were compared between the two groups. Results After treatment, there was no signifi cant diff erence between the two groups in implant abscission rate, retention rate, mobility and gingival attachment width (P>0.05). The PLI, PD and GI of the two groups decreased signifi cantly, and the PD of the observation group was lower than that of the control group (P<0.05). There was no significant difference in PLI and GI between the two groups (P>0.05). At 3 months and 1 year after operation, the buccal lingual bone absorption and proximal distal bone absorption in the observation group were lower than those in the control group (P<0.05). After treatment, the levels of IL-1β, TNF-α, CRP and other infl ammatory factors in the two groups were signifi cantly lower than those before treatment, and the levels in the observation group were lower than those in the control group (P<0.05). Conclusion The eff ect of GBR on periodontitis patients with tooth defect is signifi cant, which can improve the marginal bone resorption of implant in a short time, maintain the stability of periodontal status and reduce the infl ammatory reaction of periodontal tissue.
