摘要:
目的：观察A型肉毒毒素联合超脉冲点阵CO2激光治烧伤修复后瘢痕的疗效及安全性。方法：收集2016年2月-2021年2 月笔者医院诊治的80例烧伤修复后瘢痕患者，依据随机数字法分为观察组与对照组，每组40例。对照组予以超脉冲点阵CO2 激光治疗，观察组在对照组治疗基础上予以A型肉毒毒素治疗，选择温哥华瘢痕评分量表评估两组患者瘢痕恢复状况，比较 其瘢痕厚度、转化生长因子-β1（TGF-β1）以及完全上皮化时间的变化，采用视觉模拟评分法对其疼痛状况改善加以评估， 并通过观察两组不良反应的发生率评价其安全性。结果：治疗后，观察组VSS评分及瘢痕厚度明显低于对照组，观察组 TGF-β1 水平显著低于对照组，差异均有统计学意义（P＜0.05）；治疗后，观察组完全上皮化时间及疼痛VAS评分皆明显 低于对照组，差异均有统计学意义（P＜0.05）；两组不良反应发生率比较，差异无统计学意义（P＞0.05）。结论：A型肉 毒毒素联合超脉冲点阵CO2激光治疗烧伤修复后瘢痕疗效显著且安全，可加快瘢痕组织消退，缓解痒痛症状，值得临床推广 应用。
Abstract:
Objective To analyze the effi cacy and safety of botulinum toxin a combined with ultra pulsed fractional CO2 laser in the treatment of scar after burn repair. Methods 80 patients with scar after burn repair treated in our hospital from February 2016 to February 2021 were randomly divided into observation group and control group, with 40 cases in each group. The control group was treated with ultra pulse fractional CO2 laser therapy, and the observation group was treated with botulinum toxin A on the basis of the control group; Vancouver scar scale (VSS) was selected to evaluate the recovery of patients’ scars, and the scar thickness and transformation growth factors were compared-β1 (TGF-β1) The improvement of pain was evaluated by visual analogue scale (VAS), and its safety was confi rmed by the incidence of adverse reactions in the two groups. Results After treatment, the VSS score and scar thickness in the observation group were signifi cantly lower than those in the control group (P ＜0.05). TGF -β was signifi cantly lower than that in the control group (P＜0.05). The time of complete epithelialization and the VAS score in the observation group were signifi cantly lower than those in the control group (P ＜0.05). There was no signifi cant diff erence in the total incidence of adverse reactions between the observation group and the control group (P ＞0.05). Conclusion Botulinum toxin a combined with ultra pulsed fractional CO2 laser is safe and eff ective after burn repair. It can accelerate the regression of scar tissue and alleviate itching and pain symptoms. It is worthy of popularization and application.
