摘要:
[摘要]目的:探究超声导入曲安奈德联合微等离子体射频技术对增生性瘢痕患者的瘢痕修复效果。方法:选择2016年3
月-2019年5月笔者医院医学美容科和烧伤整形科收治的90例增生性瘢痕患者,通过随机数字表法分为对照组(n =45)和观察
组(n=45)。两组均予以微等离子体射频技术治疗,观察组在此基础上予以超声导入曲安奈德进行治疗。比较两组疼痛及瘙
痒症状、瘢痕恢复情况、心理状况、疗效、不良反应及瘢痕组织学特征变化。结果:治疗后,两组温哥华瘢痕评分量化表
(VSS)、疼痛、瘙痒评分较治疗前降低,且观察组低于对照组(P <0.05)。治疗后,观察组患者的焦虑、抑郁评分较治疗
前降低,主观幸福感评分较治疗前升高(P <0.05);对照组患者的抑郁评分较治疗前降低(P <0.05),观察组焦虑、抑郁
评分低于对照组,主观幸福感评分高于对照组(P <0.05)。观察组总有效率为91.11%,高于对照组的66.67%(P <0.05)。
组织学观察结果显示,治疗后观察组相较对照组瘢痕组织表皮恢复更为显著,真皮浅层成纤维细胞及胶原纤维数量更少且排
列相对规整。观察组、对照组仅发生色素沉着,分别为3例(6.67%)、5例(11.11%),不良反应发生率比较无统计学差异
(P >0.05)。结论:在微等离子体射频技术的基础上予以超声导入曲安奈德可有效修复增生性瘢痕患者瘢痕状态,改善瘢
痕组织学特征,减轻患者疼痛瘙痒症状,进而改善其负性心理,值得临床推广使用。

Abstract:

Abstract:Objective To investigate the effect of ultrasonic importing triamcinolone acetonide combined microplasma
radiofrequency on scar histology in the patients with hypertrophic necrosis. Methods Between March 2016 and May 2019,
90 patients with hypertrophic necrosis were randomly divided into the control group (n =45) and the observation group
(n =45). The two groups were treated with microplasma radiofrequency, and the observation group was treated with ultrasonic
importing triamcinolone acetonide.The symptoms of pain and pruritus, scar recovery, psychological status, scar histology,
adverse reactions and curative effect were compared between the two groups. Results After the treatment, the scores of the
vancouver scar scale (VSS), pain and itching in the two groups were lower than that before the treatment, and those in the
observation group were lower than that in the control group (P <0.05). After treatment, the scores of anxiety and depression in
the observation group were lower than those before treatment, the scores of subjective well-being was higher than that before
treatment (P <0.05), the score of depression in the control group was lower than that before treatment (P <0.05), the scores of
anxiety and depression in the observation group were lower than those in the control group, and the scores of subjective wellbeing
in the observation group were higher than those in the control group (P <0.05). The results of the histological observation
show that the skin of the dermal fibroblast and the collagen fibers in the dermis is less and the arrangement is relatively regular
compared with that of the control group compared with the control group. The total effective rate in the observation group was
91.11% higher than that in the control group (66.67%, P <0.05). Only 3 cases (6.67%) and 5 cases (11.11%) of pigmentation
occurred in the observation group and the control group respectively, and there was no statistical difference in the incidence
of adverse reactions (P >0.05). Conclusion On the basis of microplasma radiofrequency, ultrasonic importing triamcinolone
acetonide can effectively repair the scar of patients with hypertrophic necrosis, improve that histological characteristic of the
scar, reduce the pain and itching symptoms of patients, and then improve their negative psychology, which is worthy for clinical
application.
