摘要:
目的：探讨5-氨基酮戊酸光动力疗法（5-aminolevulinic acid photodynamic therapy，ALA-PDT）治疗中重度痤 疮患者的临床疗效。方法：采用随机数字表法将2021年1月-2022年1月笔者医院收治的中重度痤疮患者80例分为对照组 （n=40）和治疗组（n=40），对照组予以红蓝光照射治疗，治疗组采取ALA-PDT治疗，均持续治疗4周。比较两组临床疗 效，治疗前后痤疮严重程度分级系统评分（Global Acne grading system，GAGS）、痤疮特异性生活质量量表评分（The acne-specific quality of life questionnaire，Acne-QOL）、皮损情况、炎症后红斑情况及不良反应发生情况。 结果：治疗组临床总有效率为95.00%，高于对照组的72.50%（P＜0.05）；与治疗前相比，治疗后两组GAGS评分均降低 （P＜0.05），两组Acne-QOL量表评分均升高（P＜0.05），两组炎性皮损和非炎性皮损数量均减少（P＜0.05），两组红斑 指数和红斑主观评分均降低（P＜0.05），且治疗组GAGS评分、炎性皮损和非炎性皮损个数、红斑指数和红斑主观评分均低 于对照组（P＜0.05），Acne-QOL量表评分高于对照组（P＜0.05）；治疗组不良反应总发生率低于对照组（P＜0.05）。结 论：中重度痤疮患者采用ALA-PDT治疗效果显著，可降低患者痤疮严重程度，提高生活质量，改善皮损情况及炎症后红斑， 值得推广。
Abstract:
Objective To investigate the eff ects of 5-aminopentulic acid photodynamic therapy (ALA-PDT) on infl ammatory skin lesions and erythema after infl ammation in patients with moderate and severe acne. Methods 80 patients with moderate and severe acne who were treated in the author’s hospital from January 2021 to January 2022 were divided into control group (n=40) and treatment group (n=40) by random number table method. The control group was treated with red and blue light irradiation, and the treatment group was treated with ALA-PDT for 4 weeks. The clinical effi cacy, Global acne severity grading system (GAGS), acne-specifi c quality of life Scale (Acne-QOL), skin lesions, erythema after infl ammation and the occurrence of adverse reactions were compared between the two groups before and after treatment. Results The total eff ective rate of the treatment group was 95.00%, which was higher than that of the control group 72.50%(P＜0.05). After treatment, compared with before treatment, GAGS scores and Acne-QOL scale scores in both groups increased (P＜0.05), the number of infl ammatory skin lesions and non-infl ammatory skin lesions and erythema index and subjective erythema score in both groups decreased (P＜0.05). In the treatment group, GAGS scores, number of infl ammatory and non- infl ammatory skin lesions, erythema index and subjective erythema score were lower than those in the control group (P＜0.05), and Acne QOL scale scores was higher than that in the control group (P＜0.05). The total incidence of adverse reactions in treatment group was lower than that in control group (P＜0.05). Conclusion AlA-PDT can signifi cantly reduce the severity of acne, improve the quality of life, improve the condition of skin lesions and erythema after infl ammation in patients with moderate and severe acne, which is worth promoting.
