摘要:
目的：探讨自拟清热解毒汤联合针灸治疗湿热蕴结型痤疮的疗效及对患者皮肤屏障功能的影响。方法：选取80例2019年8月-2020年8月笔者医院皮肤科收治的寻常痤疮患者作为研究对象，通过随机数字表法分为试验组（40例）和对照组（40例）。对照组给予异维A酸软胶囊、莫匹罗星软膏进行治疗，试验组在对照组的基础上给予自拟清热解毒汤联合针灸疗法进行治疗，两组均持续治疗4周。比较两组治疗前、治疗4周后的中医证候积分、皮损评分、皮肤屏障功能及血清炎性因子水平。结果：与治疗前比较，治疗4周后，两组颜面油腻、皮疹脓疱、尿黄、口臭评分、中医证候总分、肿痛、质地、颜色、结节、囊肿评分、经皮水分丢失量、皮肤粘脱蛋白质含量、血清白介素-6（IL-6）、肿瘤坏死因子-α（TNF-α）、C反应白蛋白（CRP）水平均降低，试验组低于对照组，差异有统计学意义（P＜0.05）；而两组角质层含水量均升高，试验组高于对照组（P＜0.05）。结论：自拟清热解毒汤联合针灸疗法可明显改善寻常痤疮患者中医证候及皮损症状，并可增强患者皮肤屏障功能，减轻机体炎症反应，疗效显著。
Abstract:
Objective To explore the therapeutic effect of self-made Qingrejiedu decoction combined with acupuncture on damp-heat accumulating acne and its effect on the skin barrier function of patients. Methods 80 patients with acne vulgaris admitted to the department of dermatological in our hospital from August 2019 to August 2020 were selected as the research objects, and they were divided into the test group (40 cases) and the control group (40 cases) by a random number table method. The control group was treated with isotretinoin soft capsule and mupirocin Ointment. The test group was treated with self-made Qingrejiedu decoction combined with acupuncture and moxibustion therapy on the basis of the control group. Both groups were treated for 4 weeks. The scores of traditional chinese medicine syndrome, skin lesion, skin barrier function and the level of serum inflammatory factor before and after 4 weeks of treatment between the two groups were compared. Results Compared with before treatment, after 4 weeks of treatment, the scores of had greasy face, rash pustules, yellow urine, bad breath, the total score of traditional chinese medicine syndrome, the scores of swelling and pain, texture, color, nodules, cyst, the transdermal water loss, skin adhesion protein content, the levels of serum interleukin-6 (IL-6), tumor necrosis factor- α (TNF- α), C-reactive albumin (CRP) in the two groups decreased, the test group was lower than the control group, the difference was statistically significant(P＜0.05).The water content of the stratum corneum in the two groups increased, the test group was higher than the control group (P＜0.05). Conclusion Self-made Qingrejiedu decoction combined with acupuncture and moxibustion therapy could significantly improve the traditional chinese medicine syndrome and skin lesions of patients with acne vulgaris, and could enhance the skin barrier function of patients and reduce the body’s inflammatory response. The efficacy was significant.
