摘要:
目的:探讨即刻种植联合富血小板纤维蛋白(Platelet-rich fibrin,PRF)对前牙缺失患者牙槽骨吸收及龈沟液骨代 谢指标的影响。方法:选择2018年3月-2020年3月笔者医院就诊的106例前牙缺失患者为研究对象,予以随机数字表法分为对 照组(即刻种植)53例和观察组(即刻种植联合PRF)53例,记录两组患者的临床疗效及术后不良反应情况;比较治疗后龈 沟液骨桥蛋白(Osteopontin,OPN)、碱性磷酸酶(Alkaline phosphatase,ALP)及骨保护素(Osteoprotegerin,OPG)水 平变化;随访12个月,记录两组患者术后种植体骨密度、唇腭向骨高度、唇腭向骨厚度、牙槽骨吸收及美学效果。结果: 对照组显效28例,有效13例,无效12例;观察组显效37例,有效10例,无效3例,对照组和观察组总有效率依次为77.35%、 94.34%,差异有统计学意义(P<0.05)。观察组唇腭向骨高度、厚度及种植体周围骨密度均高于对照组,牙槽骨吸收情况 低于对照组,差异有统计学意义(P<0.05)。术后观察组OPG水平、美学评分均较对照组高,OPN、ALP低于对照组,差异有 统计学意义(P<0.05)。观察组并发症发生率5.66%低于对照组的16.98%,差异有统计学意义(P<0.05)。结论:即刻种 植联合PRF对前牙缺失患者临床疗效显著,可减少牙槽骨吸收,改善骨代谢,降低不良反应发生率。
Abstract:
Objective To investigate the eff ect of immediate implantation combined with platelet-rich fi brin (PRF) on alveolar bone resorption and gingival crevicular fl uid bone metabolism in patients with anterior tooth loss. Methods The 106 patients with missing anterior teeth who were treated in our hospital from March 2018 to March 2020 were selected as the research objects. According to the random number table, they were divided into control group (immediate implantation) 53 cases, and observation group (immediate implantation combined with PRF) 53 cases, the clinical effi cacy and postoperative adverse reactions of the two groups were recorded; the gingival crevicular fl uid osteopontin (OPN), alkaline phosphatase (ALP), andosteoprotegerin (OPG) were compared after treatment.Level changes,follow up for 12 months, record the postoperative implant bone density, lip and palate bone height, lip and palate bone thickness, alveolar bone resorption, and aesthetic eff ects of the two groups of patients. Results In the control group, 28 cases were markedly eff ective, 13 cases were eff ective, and 12 cases were ineff ective. In the observation group, 37 cases were markedly eff ective, 10 cases were eff ective, and only 3 cases were ineff ective. The total eff ective rates of the control group and the observation group were 77.35% and 94.34%, respectively, with statistical signifi cance (P<0.05). The height and thickness of labial and palatal bone and the bone density around the implant in the observation group were higher than those in the control group, and the alveolar bone resorption in the observation group was lower than that in the control group, the difference was statistically signifi cant (P<0.05). The OPG level and aesthetic score of the observation group were higher than those of the control group, and the OPN and ALP of the observation group were lower than those of the control group (P<0.05).The incidence of complications in the observation group was 5.66%,lower than 18.87% in the control group, the diff erence was statistically signifi cant(P<0.05). Conclusion Immediate implantation combined with PRF has a signifi cant clinical eff ect on patients with missing anterior teeth, which can reduce alveolar bone resorption, improve bone metabolism, and reduce the incidence of adverse reactions.
