摘要:
目的:探讨不同密度高能量脉冲CO2激光修复面部痤疮凹陷性瘢痕的效果及安全性。方法:遴选2019年4月-2021年4月 复旦大学附属中山医院吴淞医院皮肤科就诊的122例痤疮凹陷性瘢痕患者,随机分为高密度组(60例采取高密度高能量脉冲 CO2激光修复治疗)和低密度组(62例采用低密度高能量脉冲CO2激光修复治疗),比较两组临床疗效、治疗安全性、疼痛程 度、不良反应发生情况及患者美学满意度。结果:治疗后,低密度组瘢痕改善总有效率显著高于高密度组(93.55% vs 78.33%, P<0.05);治疗后,低密度组水肿持续时间、脱痂时间、色素沉着持续时间均显著小于高密度组(P<0.05); 各组内治疗前后VAS评分对比,差异有统计学意义(P<0.05),治疗后各时间点低密度组评分均显著低于高密度组(P<0.05); 低密度组总并发症率显著低于高密度组(6.45% vs 18.33%,P<0.05);低密度组美学满意度显著高于高密度组(85.48% vs 68.34%,P<0.05)。结论:低密度高能量脉冲CO2激光较高密度高能量修复面部痤疮凹陷性瘢痕疗效更好,恢复更快, 相关并发症更少,美学满意度更高,值得临床推广。
Abstract:
Objective To investigate the effect and safety of different density high-energy pulsed CO2 lasers on repairing facial acne depressed scars. Methods 122 patients with acne depressed scars admitted to department of dermatology of Wusong Hospital of Zhongshan Hospital Affi liated to Fudan University were selected between April 2019 and April 2021, and they were randomly divided into high-density group (60 cases treated with high-density high-energy pulsed CO2 laser repair treatment) and low-density group (62 cases receiving low-density high-energy pulsed CO2 laser repair treatment). The clinical effi cacy of scar improvement, duration of edema, decrustation time, duration of post-infl ammatory hyperpigmentation (PIH), score of pain visual analogue scale (VAS), occurrence of adverse reactions and cosmetic satisfaction were compared before and after treatment. Results After treatment, the overall eff ective rate of scar improvement in the low-density group was signifi cantlyhigher than that in the high-density group (93.55% vs 78.33%, P<0.05). After treatment, the duration of edema, descabling time, The duration of pigmentation was significantly less than that of the high-density group (P<0.05). The VAS scores before and after treatment in each group were statistically significant (P<0.05), and the scores of the low-density group were significantly lower than those of the high-density group (P<0.05) at all time points after treatment. The total complication rate of the low-density group was signifi cantly lower than that of the high-density group (6.45% vs 18.33%, P<0.05). the aesthetic satisfaction of the low-density group was signifi cantly higher than that of the high-density group (85.48% vs 68.34%, P<0.05). Conclusion Compared with high-density high-energy pulsed CO2 laser, low-density high-energy pulsed CO2 laser has a better effi cacy on repairing facial acne depressed scars, with faster recovery, fewer complications and higher cosmetic satisfaction.
