[摘要]目的:观察强脉冲光(IPL)与红蓝光分别联合果酸治疗寻常性痤疮的临床疗效,并进行对比分析。方法:选择2016
年6月-2016年8月笔者科室就诊的40例面部寻常性痤疮患者,随机分为两组。常规组:20例,给予红蓝光联合果酸治疗;研
究组:20例,给予强脉冲光联合果酸治疗。对比两组治疗后皮损减退、色素沉着、并发症及血清中IL-8、IL-17水平情况。
结果:治疗2周后,研究组皮损减退总有效率35.00%,显著高于常规组5.00%,差异有统计学意义(χ2=3.906,P =0.048);
治疗6周后,研究组皮损减退总有效率55.00%,显著高于常规组20.00%,差异有统计学意义(χ2=5.227,P =0.022);治
疗8周后,研究组皮损减退总有效率85.00%,明显高于常规组45.00%,差异有统计学意义(χ2= 7.033,P =0.008)。治疗
2周后,研究组色素沉着总有效率40.00%,显著高于常规组10.00%,差异有统计学意义(χ2=4.800,P =0.029);治疗6周
后,研究组色素沉着总有效率60.00%,显著高于常规组25.00%,差异有统计学意义(χ2=5.013,P =0.025);治疗8周后,
研究组色素沉着总有效率85.00%,显著高于常规组50.00%,差异有统计学意义(χ2=5.584,P =0.018)。治疗8周后,研
究组IL-8水平(8.76±1.26)显著低于常规组(10.96±1.62),差异有统计学意义(t =4.794,P <0.05);研究组IL-17
水平(13.69±2.63)显著低于常规组(17.62±3.21),差异有统计学意义(t =4.235,P <0.05);研究组总并发症发生
率为10.00%,常规组总并发症发生率为20.00%,研究组并发症发生率低于常规组,但两组患者并发症发生率无显著性差异
(χ2=0.7843,P =0.3758)。结论:寻常性痤疮患者给予强脉冲光联合果酸治疗,其疗效较好,不良反应较少。

Abstract:
Abstract: Objective To observe the clinical efficacy of intense pulsed light versus red-blue light combined with alpha-hydroxy
acid (AHA) in the treatment of moderate to severe inflammatory acne. Methods 40 cases of facial inflammatory acne patients
in our hospital from June 2016 to August 2016 were selected and randomly divided into 2 groups, 20 cases of the conventional
group were treated with red-blue light combined with AHA, 20 cases of the study group received intense pulsed light combined
with AHA treatment, then the postoperative hypopigmentation, pigmentation, complications and serum IL-8,IL-17 levels
were compared. Results The efficacy rate of the hypopigmentation in the study group at post 2 weeks was higher than that
of the conventional group (35.00% vs 5.00%,χ2=3.906,P =0.048); The study group’s hypopigmentation efficiency after 6
weeks of treatment was 55%, which was evidently higher than 20.00% of the conventional group, the difference had statistical
meaning (χ2=5.227, P =0.022); The study group’s hypopigmentation efficiency after 8 weeks of treatment was 85.00%, which
was evidently higher than 45.00% of the conventional group, the difference had statistical meaning (χ2=7.033,P =0.008);The
study group’s pigmentation efficiency at 2 weeks after treatment was 40.00%, which was apparently higher than 10.00% of
the conventional group, the difference had statistical meaning (χ2=4.800, P =0.029); That of the study group after 6 weeks of
treatment was 60.00%, which was sharply higher than that of the conventional group 40.00%, the difference had statistical
points (χ2=5.227,P =0.022); That of the study group after 12 weeks of treatment of 85.00% was evidently higher than that
of the conventional group 50.00%, the difference showed statistical significance (χ2=5.584,P =0.018); The study group’sIL-
8level at 8 weeksafter treatment (8.76±1.26) was sharply lower than that ofthe conventionalgroup (10.96±1.62), the difference
had statistical significance (t =4.794, P <0.05); The study group’s IL-17 level (13.69±2.63) was evidently lower than that
ofthe conventionalgroup (17.62±3.21), the difference showed statistical meaning (t =4.235, P <0.05). The incidence rate
of the complications of the study group was lower than that of the conventional group, with no statistic difference (10% vs
20%,χ2=0.7843, P =0.3758). Conclusion Patients with mild, moderate and severe inflammatory acne can be treated with
intense pulsed light combined with AHA, which gives better curative effect as well as less adverse reactions.
