摘要:
目的:探究光动力(Photodynamic therapy,PDT)联合点阵CO2激光治疗增生性瘢痕(Hypertrophic scar,HS)临 床效果。方法:选择2017年8月-2019年8月来笔者医院就诊的HS患者124例,随机分成观察组和对照组,各62例。对照组采 用点阵CO2激光治疗,观察组在对照组基础上联合光动力治疗,两组均治疗4个月,并随访1年。比较两组患者的临床疗效及 复发率,比较治疗前、后,两组患者的瘢痕状况[温哥华瘢痕量表(Vancouver scar scale,VSS)、北卡罗来纳大学(the University of North Carolina,UNC)瘢痕量表]、转化生长因子-β1(Transforming growth factor-β1,TGF-β1)、 骨形态发生蛋白-7(BMP-7)、表皮生长因子(Epidermis growth factors,EGF)水平、心理水平[焦虑(Self-rating anxiety scale,SAS)自评量表、抑郁(Self-rating depression scale,SDS)自评量表]、生活质量评分(Dermatology life quality index,DLQI),并观察两组不良反应发生情况。结果:治疗后,观察组的总有效率为93.55%,高于对照组 的79.03%(P<0.05);观察组的复发率为6.45%,低于对照组的19.35%(P<0.05);治疗后,两组的VSS、UNC评分均低 于治疗前,且观察组均低于对照组(P<0.05);治疗后,两组TGF-β1、EGF水平均低于治疗前,且观察组均低于对照组 (P<0.05),两组BMP-7均高于治疗前,且观察组高于对照组(P<0.05);治疗后,两组的SAS、SDS、DLQI评分均低于 治疗前,且观察组均低于对照组(P<0.05);治疗期间,观察组及对照组的不良反应发生率分别为12.90%、11.29%,差异 无统计学意义(P>0.05)。结论:PDT联合点阵CO2激光治疗HS患者疗效好,能有效改善HS患者瘢痕症状及TGF-β1、EGF、 BMP-7水平,其心理及生活质量均得到改善,具有较好安全性。
Abstract:
Objective To explore the clinical eff ect of photodynamic therapy(PDT) combined with fractional CO2 laser in the treatment of hypertrophic scar(HS). Methods A total of 124 HS patients treated in the hospital from August 2017 to August 2019 were enrolled and randomly divided into observation group and control group, 62 cases in each group. The control group was treated with fractional CO2 laser, while observation group was treated with PDT on basis of control group. Both groups were treated for 4 months and followed up for 1 year. The clinical curative effect and recurrence rate after treatment, scars status [Vancouver scar scale(VSS), University of North Carolina(UNC)scar scale], levels of transforming growth factor-β1 (TGF-β1), bone morphogenetic protein-7(BMP-7)and epidermal growth factor(EGF), and psychological status [Selfrating anxiety scale(SAS), Self-rating depression scale(SDS), Dermatology life quality index(DLQI)] before and after treatment were compared between the two groups. The occurrence of adverse reactions during treatment in both groups was observed. Results After treatment, total response rate of observation group was higher than that of control group (93.55% vs 79.03%, P<0.05). After 1 year of follow-up, recurrence rate of observation group was lower than that in control group (6.45% vs 19.35%, P<0.05). After treatment, scores of VSS and UNC scar scale in both groups were decreased, which were lower in observation group than control group(P<0.05). After treatment, levels of TGF-β1 and EGF in both groups were decreased, which were lower in observation group than control group (P<0.05), and BMP-7 in both groups was increased, which was higher in observation group than control group(P<0.05). After treatment, scores of SAS, SDS and DLQI in both groups were decreased, which were lower in observation group than control group (P<0.05). During treatment, the difference in incidence of adverse reactions between observation group and control group was not statistically significant (12.90% vs 11.29%, P>0.05). Conclusion The curative effect of PDT combined with fractional CO2 laser is good on HS patients. It can effectively improve scar symptoms, TGF-β1, EGF and BMP-7 levels. Improve psychology and quality of life, with high safety.
